右美托咪啶预处理对肺癌根治术患者肺功能的影响 被引量:2

The influence of dexmedetomidine pretreatment for the pulmonary function in patients undergoing radical surgery for lung cancer

摘要 目的:观察右美托咪啶对肺癌根治术患者肺功能的影响。方法:选取60例择期行肺癌根治术患者,随机分为实验组和对照组,实验组采用右美托咪啶的方法进行预处理,对照组采用安慰剂生理盐水进行预处理。两组患者分别于单肺通气后30 min(T1)、单肺通气后90 min(T2)、术后1 d(T3)三个时点采集静脉血检测白细胞介素-1β(IL-1β)、白细胞介素-10(IL-10)浓度,同时采集动脉血行血气分析。结果:与对照组比较,实验组T2,T3时点IL-1β显著降低(P<0.05),IL-10显著升高(P<0.05)。实验组患者术后动脉血氧分压(Pa O2)显著高于对照组,而二氧化碳分压(Pa CO2)显著低于对照组(P<0.05)。结论:右美托咪啶预处理可抑制促炎性细胞因子的表达,促进抗炎性细胞因子的释放,且能改善患者术后肺交换功能,对患者肺功能有一定保护作用。 Objective:To observe the effect of dexmedetomidine on pulmonary function in patients undergoing radical sur-gery for lung cancer. Methods:Sixty patients undergoing radical resection of lung cancer were randomly divided into experimen-tal group and control group,the experimental group method using dexmedetomidine pretreatment,the control group using saline placebo pretreatment. The interleukin - 1β(IL - 1 β),interleukin - 10(IL - 10)of two group was detected in 30 min after one lung ventilation(T1),90 min after one lung ventilation(T2),1 postoperative days(T3)three point,and arterial blood gas analysis . Results :Compared with the control group ,T 2 ,T 3 IL - 1 β in the experimental group decreased significantly( P 〈0. 05),IL - 10 increased significantly(P 〈 0. 05). The alveolar oxygen partial pressure(PaO2 )of experimental group was sig-nificantly higher than that in control group after surgery,while alveolar partial pressure of carbon dioxide(PaCO2 )was signifi-cantly lower than the control group(P 〈 0. 05). Conclusion:Dexmedetomidine pretreatment can inhibit the expression of proin-flammatory cytokines,promote anti - inflammatory cytokine release,and can improve the postoperative lung function exchange, has certain protective effect on lung function in patients.
