经伤椎固定与传统短节段固定后腰椎骨折模型的生物力学稳定性比较 被引量:8

摘要 目的比较经伤椎固定与传统短节段固定后腰椎骨折模型的生物力学稳定性,为经伤椎固定的临床应用提供实验依据。方法选取6具市售新鲜家猪L1~5脊柱标本。每具标本均设有完整、4钉固定和6钉固定状态。先进行完整状态下的稳定性测试,再采用预损伤结合逐级撞击法制造L3椎体骨折模型,钉棒固定标本后,再次进行稳定性测试。得到脊柱标本在屈伸、侧屈、旋转运动方向加载状态下的运动范围(ROM)。比较3种状态下脊柱标本ROM的差异。结果 4钉固定状态脊柱的ROM除了在旋转运动方向与完整状态的ROM比较差异无统计学意义(P〉0.05)外,在屈伸和侧屈运动方向均小于完整状态(P〈0.05)。在屈伸和旋转运动方向,6钉固定状态脊柱的ROM均小于4钉固定状态(P〈0.01);然而,在侧屈运动方向,6钉固定状态与4钉固定状态脊柱的ROM比较差异无统计学意义(P〉0.05)。结论在屈伸和旋转运动方向,经伤椎固定与传统短节段固定相比,具有更强的生物力学稳定性;在侧屈运动方向,两者的生物力学稳定性相当。
