辛伐他汀联合氯沙坦治疗原发性高血压合并左室肥厚的临床观察

Clinical observation of simvastatin combined with losartan in the treatment of primary hypertension complicated with left ventricular hypertrophy

目的探讨辛伐他汀与氯沙坦联合应用对原发性高血压合并左室肥厚患者的治疗效果。方法 90例原发性高血压合并左室肥厚患者随机分为观察组和对照组,每组45例。观察组予以辛伐他汀联合氯沙坦治疗,对照组予以氯沙坦治疗。比较两组治疗前后血压及舒张末期左室后壁厚度(LVPWT)、舒张末期室间隔厚度(IVST)、舒张末期左室腔内径(LVDd)和左心室质量指数(LVMI)的变化情况。结果两组治疗后收缩压及舒张压水平比治疗前明显下降,差异具有统计学意义(P<0.05),观察组治疗后收缩压及舒张压水平明显低于对照组(P<0.05)。两组治疗后LVPWT、IVST、LVDd及LVMI水平比治疗前明显下降,差异具有统计学意义(P<0.05);观察组治疗后LVPWT、IVST、LVDd及LVMI水平均明显低于对照组(P<0.05)。结论在应用氯沙坦基础上联合辛伐他汀治疗,可显著提高对原发性高血压合并左室肥厚患者的治疗效果,值得临床推广应用。

Objective To investigate the curative effect of simvastatin combined with losartan in the treatment of primary hypertension complicated with left ventricular hypertrophy.Methods A total of 90 patients of primary hypertension complicated with left ventricular hypertrophy were randomly divided into observation group and control group, with 45 cases in each group. The observation group received simvastatin combined with losartan for treatment, and the control group received losartan alone. Comparisons were made on the changes of blood pressure, diastasis left ventricular posterior wall thickness （LVPWT）, diastasis interventricular septum thickness （IVST）, diastasis left ventricular diastolic diameter （LVDd）, and left ventricular mass index （LVMI） before and after treatment in the two groups.Results After treatment, both groups had decreased systolic pressure and diastolic pressure levels compared with those before treatment, and the difference had statistical significance （P〈0.05）. Those in the observation group were all lower than the control group after treatment （P〈0.05）. Both groups had decreased LVPWT, IVST, LVDd, and LVMI levels after treatment than those before treatment, and the difference had statistical significance （P〈0.05）. The observation group had much lower levels of LVPWT, IVST, LVDd, and LVMI than the control group after treatment （P〈0.05）.Conclusion Combined application of simvastatin and losartan can remarkably improve curative effect for primary hypertension complicated with left ventricular hypertrophy, and this method is worthy of clinical promotion and application.
