摘要
脓毒症是PICU的常见死亡原因,但目前临床上在脓毒症诊断、严重程度分级及预后评估等方面尚缺乏敏感性和特异性均较高的生物标志物。可溶性尿激酶型纤溶酶原激活物受体(solubleurokinaseplasminogenactivatorreceptor,suPAR)可参与机体纤溶酶原激活、炎症反应以及多种免疫细胞的迁移、黏附、趋化、信号传导等,是近年来发现的一种新型炎症激发受体。相关研究表明,血清suPAR水平在感染性疾病显著升高。本文就suPAR检测在脓毒症诊断及预后评估中的作用进行综述,期能寻找客观、便捷、可靠的生物标志物对脓毒症患儿进行实时评估并筛选出适合入住PICU的患儿,使其得到及时有效的救治,改善预后,提高生存率。
Sepsis is the leading cause of death in critically ill children,but it lack biomarkers with a higher sensitivity and specificity for sepsis diagnosis, severity assessment, and prognostication. Soluble uroki- nase plasminogen activator receptor( suPAR), as a novel receptor stimulated by inflammation, is involved in the plasminogen activating pathways, inflammation and migration, adhesion, chemotaxis, signal transduction of various immune cell. suPAR serum levels have been shown to significantly increase in various infectious dis- eases. This review provided a descriptive overview of the role of suPAR detection in sepsis judgment and prognostication, hoping to find a perfect biomarker for timely evaluating of children with sepsis and screening for children admitted to the intensive care unit in order to improve outcome and reduce mortality.
