MASCC危险指数评分和降钙素原应用于血液病化疗后粒细胞缺乏伴发热的价值 被引量:5

摘要 目的:评估癌症患者支持治疗的多国协作组织(MASCC)危险指数评分和降钙素原(PCT)在预测血液病化疗后粒细胞缺乏伴发热(FN)患者感染并发症的价值。方法:对2013年1月至2014年6月我院血液病房化疗后180例FN患者的资料进行回顾性分析,分析MASCC危险指数评分和PCT值与细菌血症和感染性休克的相关性。结果:180例患者中有20例(11.1%)发展为细菌血症,14例(7.8%)发展为感染性休克。PCT≥0.5 ng/m L,MASCC危险指数评分<21分是细菌血症发生的独立危险因素;PCT≥2 ng/m L和MASCC危险指数评分<21分是感染性休克的独立危险因素。由MASCC危险指数评分区分的156例低风险患者,30例患者PCT≥0.5 ng/m L,其中8例(26.7%)发展为细菌血症;16例患者PCT≥2 ng/m L,其中4例(25.0%)发展为感染性休克。结论 :MASCC危险指数评分与PCT联合应用,作为FN患者常规筛查将促进危险分层的划分。
