胸腰椎骨折应用微创经皮椎弓根螺钉内固定治疗的临床观察

Clinical observation on treating thoracolumbar fractures by minimally invasive percutaneous pedicle screw fixation

目的:对胸腰椎骨折应用微创经皮椎弓根螺钉内固定治疗的临床疗效进行分析。方法:选取胸腰椎骨折患者78例,根据治疗方法不同将其分为两组,以行传统开放椎弓根钉内固定术的38例患者为对照组,以行微创经皮椎弓根螺钉内固定的40例患者为观察组,分析两组患者围术期指标及治疗效果。结果:观察组手术用时及切口长度显著短于对照组,术中出血量、术后引流量显著少于对照组,术后1d VAS评分显著低于对照组。两组患者术后椎体前缘高度与后凸C0bb’s角均显著改善,且组间对比无显著差异;两组患者术后1年Oswestry功能障碍指数评分无显著差异。结论:胸腰椎骨折应用微创经皮椎弓根螺钉内固定治疗临床疗效显著,操作简便且给患者造成的创伤较小,有利于患者术后尽快恢复,值得在临床中推广。

Objective： To analyze clinical effects of minimally invasive percutaneous pedicle screw fixation on thoracolumbar fractures. Methods：78 patients were randomly divided into two groups, the observation group was given minimally invasive percutaneous pedicle screw fixation, and another was given traditional open pedicle screw fixation. Results： Surgery time, incision length, bleeding volume, postoperative drainage volume and postoperative 1d VAS score in the observation group were significantly lower. Postoperative anterior vertebral height and kyphosis C0bb＇s corner in two groups were significantly improved without significant differences. After 1 year, the Oswestry disability index score in two groups shows no significant difference.
