超声评价阴茎勃起功能障碍患者肱动脉内皮功能的研究

Evaluation of brachial artery endothelial function in patients with erectile dysfunction by ultrasonography

目的应用高分辨力超声测量无心血管疾病及其他危险因素的阴茎勃起功能障碍(ED)患者肱动脉血流介导的扩张反应的百分变化率,以评价肱动脉内皮功能。方法超声测量22例ED患者(ED组)和18例健康志愿者(对照组)静息状态下、反应性充血后及舌下含服硝酸甘油后的肱动脉内径,计算反应性充血和硝酸甘油诱发的内径百分变化率。结果 ED组与对照组肱动脉基础内径比较差异无统计学意义,ED组反应性充血诱发的肱动脉内径百分变化率低于对照组[(4.6±2.0)%vs.(11.1±3.3)%,P﹤0.01],两组含服硝酸甘油后的肱动脉内径百分变化率比较差异无统计学意义[(27.9±9.0)%vs.(28.2±8.1)%,P=0.27]。结论无心血管疾病及其他危险因素的ED患者肱动脉内皮功能减低;超声检测ED患者血管内皮功能能及早发现临床潜在的、无症状的心血管疾病,具有重要的临床意义。

Objective To evaluate brachial artery endothelial function in patients with erectile dysfunction（ED）without overt cardiovascular disease or cardiovascular risk factors. Methods Twenty-two patients with ED and eighteen healthy volunteers were enrolled into ED group and control group. The change of the brachial artery diameter induced by reactive hyperemia or sublingual nitroglycerin administration was recorded and compared with that of the normal steady state. Size of brachial artery diameter was measured with high-resolution ultrasound under the condition of normal steady state,reactive hyperemia and sublingual nitroglycerin administration. Results No significant difference was found of the diameter of brachial arteries between ED group and the control group. The percentage of diameter change in the ED group was significantly decreased compared with that of the control group[（4.6±2.0）% vs.（11.1±3.3）%,P﹤0.01]. The changes in brachial artery diameter induced by nitroglycerin was not significant between the ED group and the control group[（27.9±9.0）% vs.（28.2±8.1）%,P=0.27].Conclusion Brachial endothelial function is impaired in patients with ED without overt cardiovascular disease or cardiovascular risk factors. Evaluation of vascular endothelial function in patients with ED is helpful to find some early asymptomatic cardiovascular disease.
