摘要
目的:检测吸烟致慢性阻塞性肺疾病(COPD)患者肺组织T辅助细胞17(Th17)及调节性T细胞(Treg)分布及IL-17、IL-10表达水平变化,探讨吸烟导致的COPD慢性炎性反应的可能机制。方法:56例"孤立性肺结节"行手术切除肺叶患者的肺组织标本分为4组:吸烟COPD组、吸烟肺功能正常组、不吸烟COPD组和不吸烟肺功能正常组。收集各组患者肺组织标本,HE染色法观察肺组织形态,实时荧光定量PCR方法检测Th17特异性转录因子RORγt,Treg特异性转录因子FOXP3水平及炎性因子IL-17、IL-10 mRNA水平;ELISA法定量检测IL-17、IL-10蛋白水平。结果:吸烟COPD组气道炎症病理评分明显增高,其RORγt、IL-17 mRNA水平及IL-17均高于另外3组(P<0.05);吸烟COPD组肺组织中FOXP3水平稍低于其他三组,差异无统计学意义(P>0.05),但IL-10 mRNA及IL-10水平均显著低于另外三组(P<0.05);吸烟肺功能正常组较非吸烟肺功能正常组气道炎症评分及RORγt、IL-17 mRNA及IL-17细胞因子水平增高,IL-10 mRNA表达及IL-10水平无显著差异。结论:吸烟相关慢性阻塞性肺疾病患者肺组织中存在以Th17优势表达为主的免疫失衡,可能在吸烟相关慢性阻塞性肺疾病发病机制中发挥重要作用。
Objective To investigate the Thl7 cell and Treg cell distribution and the expression of IL-17 and IL-10 in lung tissue of patients with COPD induced with smoking, and explore the possible mechanism of chronic inflammatory reaction in smoking induced COPD. Methods Fifty-six cases of lung tissue samples from patients with solitary pulmonary nodule under- went pulmonary lobectomy were divided into 4 groups, including smoking with COPD group, smoking with normal lung func- tion group, non-smoking with COPD group, non-smoking with normal lung function group. The morphology of lung tissue was observed with HE staining. The mRNA expression of Thl7 specific transcription factor ROR~/t, Treg specific transcription factor FOXP3, IL-17 and IL-10 in lung tissue were determined by quantitative Real-time flourescence PCR. The protein ex- pressions of IL-17 and IL-10 was determined by ELISA. Results Compared with the other three groups, the airway inflam- matory pathologic score of smoking with COPD group was most obviously increased, the mRNA expression of RORγt, IL-17, and the protein expression of IL-17 were significantly highest( P 〈 0.05 ) , the level of FOXP3 in lung tissue was lower( P 〉 0. 05 ), but the mRNA and protein of IL-10 were significantly lower( P 〈 0.05 ). Smoking with COPD group showed lowest level of FOXP3 expression compared with the other three groups, and the mRNA and protein levels of IL-10 were decreased the most obviously than the other three groups. In smoking with normal lung function group, ROR3,t, IL-17 mRNA and IL-17 cytokines concentration become higher than the non-smoking with normal lung function group, however, IL-10 mRNA and IL-10 cytokines level were no difference. Conclusion Immunodissonance mainly demonstrated with predominant Th17 cells were observed in lung tissue of patients with COPD induced by smoking,which may play important role in the pathogenesis of COPD induced by smoking.
