误诊为急性阑尾炎的临床分析与对策

Clinical Analysis and Countermeasures of Misdiagnosed as Acute Appendicitis

目的:探讨误诊为急性阑尾炎的病例特点,分析误诊原因,提出应对策略。方法:回顾性分析2010年1月-2014年7月笔者所在医院误诊为急性阑尾炎的46例手术患者的临床资料。结果:误诊病例以妇科疾病(39.13%)、回盲部疾病(28.26%)和末段小肠疾病(21.74%)最为常见;82.61%病例无典型阑尾炎表现;28.26%病例临床资料不完整,19.57%病例临床思维方面存在欠缺。结论:完善的临床资料和科学的临床思维可以减少误诊的发生。

Objective：To investigate the characteristics of cases of misdiagnosed as acute appendicitis,analyze the causes of misdiagnosis,and propose appropriate countermeasures.Method：The clinical data of 46 cases of misdiagnosed as acute appendicitis from January 2010 to July 2014 were analyzed retrospectively.Result：Gynecological diseases （39.13%）,ileocecal disease （28.26%） and ileal disease （21.74%） were the most common cases of misdiagnosed as acute appendicitis.82.61%of cases did not show the typical appendicitis.The clinical data of 28.26%cases was incomplete,and the clinical thinking of 19.57%cases was lack.Conclusion：The perfect clinical data and scientific clinical thinking can reduce the incidence of misdiagnosis.
