儿童肾母细胞瘤肾摘除术术中麻醉管理研究

Anesthetic Management and Concerns of Children Nephroblastoma Resection

目的：对儿童肾母细胞瘤肾摘除术的麻醉特殊性及其关注点进行分析与探讨。方法选取18例2004年1月-2014年12月我院确诊为肾母细胞瘤且其对行肾母细胞瘤摘除术治疗的患儿，分析总结患儿手术治疗、麻醉管理等措施。结果17例患儿顺利通过手术治疗，1例患儿在手术过程中心率、血压下降，继而转至重症手术室抢救。结论研究表明，在对患儿实施肾母细胞瘤肾摘除手术治疗中，翻动与分离肿瘤时为麻醉主要关注点，对患儿下腔静脉行间断性压迫会引发低血压与大出血风险。

ObjectiveTo investigate the anesthetic particularity and anesthetic considerations of the childrens nephroblastoma （Wilms’tumor） resection.Methods By retrospective analysis the 18 patient＇s medical records who had been diagnosed Wilms’tumor and received surgical treatment from January 2004 to December 2014 in our hospital, analyzed and summarized the children surgery, anesthesia management and other measures.Results Seventeen patients finished the tumor resection under general anaesthesia, one patient carried out cardio-pulmonary resuscitation for severe hypotension and then carried to SICU.ConclusionThe study showed that the main anesthetic considerations of Wilms’tumor resection are severe hypotension, bleeding and potential when removing and turning over the tumor.
