摘要
目的探讨子宫内膜癌中雌激素受体(ER)、孕激素受体(PR)、表皮生长因子受体2(Cerb B-2)及增殖细胞核抗原(Ki-67)的表达及其与临床、病理特征的关系。方法选取2008年1月至2012年11月在河北联合大学附属医院妇科收治的90例手术切除子宫内膜癌患者,应用免疫组织化学方法测定ER、PR、Cerb B-2及Ki-67在不同病理分期、组织类型、分化程度和浸润转移等的表达情况。结果子宫内膜样癌中ER、PR的阳性率高于其他组织,差异有统计学意义(P〈0.05);Ⅰ期和Ⅱ期ER、PR的阳性表达率高于Ⅲ~Ⅳ期,Ⅰ期Cerb B-2、Ki-67阳性表达率低于Ⅱ期和Ⅲ~Ⅳ期(P〈0.05);G1级Ki-67阳性率低于G2、G3级,ER、PR阳性表达率高于G3级,Cerb B-2阳性率低于G3级(P〈0.05);肌层浸润深度〉1/2的癌组织中ER、PR的阳性表达率低于无肌层浸润及肌层浸润深度≤1/2组,Ki-67的阳性表达率显著高于无肌层浸润及肌层浸润深度≤1/2组(P〈0.05);有淋巴结转移的ER、PR阳性表达率低于无淋巴转移者,Ki-67阳性表达率高于无淋巴结转移者(P〈0.05)。在90例子宫内膜癌中,Cerb B-2与Ki-67蛋白表达呈正相关(P〈0.05)。结论 Cerh B-2、Ki-67的高表达与子宫内膜癌的发生、发展、不良预后有关,联合检测有助于评价子宫内膜癌的生物学行为。
Objective To explore the expressions of estrogen receptor (ER), progesterone receptor ( PR), human epidermal growth factor receptox ( CerbB-2), Ki-67 antigeny ( Ki-67 ) antigen in endometrial carcinoma and their relationship with clinical and pathologic features. Methods A total of 90 patients with endometrial carcinoma admitted in Department of Gynecology, Affiliated Hospital of Hebei United University from Jan. 2008 to Dec. 2012 were selected as research objects. The expressions of ER ,PR, cerbB-2, Ki-67 in different pathological stages, tissue types, differentiation grades, invasion and metastasis were detected by method. Results The positive rates of ER and PR in endometrioid adenocarcinoma were higher than those in other tissues, the differences were statistically significant ( P 〈 0.05 ) ; positive expression rates of ER, PR, CerbB-2 and Ki-67 in tissues of stage I were higher than those in tissues of stage II and m-IV(P 〈 0. 05 ) ; positive expression rates of ER, PR, CerbB-2 and Ki-67 in tissues of G1 class were high- er than those in tissues of G3 ,the differences were statistically significant (P 〈 0. 05 ) ;positive expression rates of ER ,PR in tissues with depth of myometrial invasion 〉 1/2 were lower than those in tissues with no muscle invasion and the depth of myometrial invasion ≤ 1/2 ;while the positive expression rate of Ki-67 in tissues with depth of myometrial invasion 〉 1/2 was significantly higher than that in tissues with no muscle invasion and the depth of myometrial invasion ≤ group 1/2 ( P 〈 0. 05 ) ; positive expression rates of ER, PR in tissues with lymph node metastasis were lower than those in tissues without lymph node metastasis, while the positive expression rate of Ki-67 in tissues with lymph node metastasis was higher than that in tissues without lymph node metastasis ( P 〈 0. 05 ). In 90 cases of endometrial carcinoma, the expression of CerbB-2 and Ki-67 protein were positively correlated (P 〈 0. 05). Condusion High expressions of CerbB-2, Ki-67 in endometrial carci- noma are positively associated with the development and progression of endometrial cancer. The detection of CerbB-2 ,Ki-67 contributes to the evaluation of biological behaviors and prognosis of endometrial carcinoma.
