咪唑安定辅助区域麻醉合适镇静深度及用量的研究

Imidazole Valium Auxiliary Dosage of Regional Anesthesia Depth of Sedation and Appropriate Research

目的:探讨咪唑安定辅助区域麻醉所采用合适的镇静深度以及合理用量。方法:本次研究选取2012年1月-2014年1月在我院接受下腹部和下肢手术的患者80例,随机分为4组,A组20例、B组20例、C组20例、D组20例,四组病例采用硬膜外麻醉或者臂丛神经阻滞,进行常规的区域麻醉后,且效果确切后针对A组、B组、C组使用咪唑安定进行镇静,D组不使用任何镇静药物,不做任何镇静措施。结果:C组烦躁率均高于A组与B组,差异具有统计学意义,P<0.05。在注药5min后,四组HR无明显变化;MAP呈下降趋势,差异无统计学意义;SpO2存在明显下降趋势,A组、B组与C组、D组比较差异具有统计学意义,P<0.05。A组镇静用量为(0.04±0.01)mg/kg、B组为(0.05±0.04)mg/kg、C组为(0.14±0.02)mg/kg,C组用量明显高于其他三组,差异具有统计学意义,P<0.05。结论:咪唑安定是区域麻醉中使用的镇静药物,适宜的等级为OAA/SⅢ级,适合的镇静用量为(0.05±0.04)mg/kg。

Objective：To explore the imidazole adopted by appropriate calm stability assist regional anesthesia depth and reasonable consumption. Methods：This study selected from January 2012 to January 2014 in our hospital 80 cases of patients with lower abdominal and lower limbs surgery, were randomly divided into 4 groups, A group 20 cases, B group 20 cases,C group 20 cases, D group 20 cases, four groups of cases with epidural anesthesia or brachial plexus block,after doing traditional regional anesthesia,and the effect after the exact for group A, group B,group C using imidazole sedation,group D does not use any sedative drugs and do not do any calm measures. Results： Be agitated rate C group were higher than that of group A and group B, statistically significant difference, P〈0. 05. In drug injection after 5rain,four groups of cases of HR has no obvious change; MAP showing a downward trend, there was no statistically significant difference; there was obvious downtrend, SpO2 in group A, group B compared with group C, group D difference is statistically significant,P〈0. 05）. Sedation group A dosage of （0. 04±0. 01）mg/kg, group B was（0. 05±0. 04） mg/kg,group C was（0. 24±0. 02）mg/kg,dosage of C group was obviously higher than that of the other three groups, statistically significant difference, P〈0. 05）. Conclusion： Imidazole valium regional anesthesia is used in the sedative drugs, appropriate level for the OAA/S Ⅲ, suitable sedative dosage was（0. 05±0. 04） mg/kg.
