66例超声诊断甲状腺结节钙化的临床价值研究

Clinical Value Research of 66 Cases of Thyroid Nodule Calcification by Ultrasonic Diagnosis

目的:研究超声诊断甲状腺结节钙化的临床价值。方法:回顾分析90例在我院接受甲状腺手术治疗的患者资料。结果:经检查,90例甲状腺结节中钙化66例,约占73.33%;良性结节57例,约占63.33%,主要包括甲状腺腺瘤、结节性甲状腺肿、甲状腺囊肿;恶性结节33例,约占36.67%,主要为甲状腺癌;39例为单发结节,51例为多发结节。恶性甲状腺结节与良性甲状腺结节均可能出现钙化情况,但恶性所占比例显著大于良性,差异具有统计学意义(P<0.05);恶性肿瘤中出现微钙化的情况较常见,而甲状腺良性病变中以粗钙化情况更多见,两者间比较差异具有统计学意义(P<0.01),微钙化与粗钙化分别常见于恶性肿瘤与甲状腺良性病变中。结论:采用超声诊断甲状腺钙化情况对临床甲状腺疾病判断具有重要价值,为患者治疗争取宝贵时间,间接提高其生存质量,临床可推广使用。

Objective.. To study the clinical value of ultrasonic diagnosis of thyroid nodule ealcifieation~ Methods.-A de- tailed retrospective analysis was made for the clinical data of 90 patients who received thyroid operative treatment in our hospital. Results： Through inspection, among 90 cases of thyroid nodules, 66 cases were calcification, accounting for about 73. 33%; 57 cases were benign thyroid nodules, accounting for 63.33%, mainly including thyroid adenoma, nodular goiter and thyroid cyst; 33 cases were malignant nodules, accounting for about 36.67%, and were mainly thyroid carcinoma; 39 cases were solitary nodules, and 51 cases were multiple nodules. Calcification might occur in both malignant thyroid nodules and benign thyroid nodules, but occurred more in malignant thyroid nodule and the difference between them was of statistical significance （P〈0. 05）. Microcalcification often occurred in malignancies, and coarse calcification occurred more in benign thyroid lesions; and the difference between them was of statistical significance （P〈 0. 01）. Microcalcification and coarse calcification were respectively often found in malignancies and benign thyroid lesions. Conclusion.. Ultrasonic diagnosis of thyroid calcification is of important value for the clinical judgment of thyroid diseases,and can earn more precious time for patients to receive treatment and indirectly improve their quality of life. Therefore, ultrasonic diagnosis can be generalized in clinical use.
