摘要
融合术后相邻节段退变(adjacent segment degeneration,ASDeg)或症状性相邻节段退变病(adjacent segment disease,ASDis)一直是脊柱外科医生争论的焦点。在此背景下,旨在保留节段运动及阻止邻近节段退变的非融合技术即动态稳定系统迅速发展。Dynesys动态中和系统(dynamic neutralization system,Dynesys)就是腰椎后路经椎弓根动态固定的代表之一,目前已历经20年的临床实践,在取得令人鼓舞的临床疗效的同时,也存在一些争议。本文主要对Dynesys的基础研究及临床经验作简要综述。
Adjacent segment degeneration( ASDeg) or adjacent segment disease( ASDis) after fusion has always been the controversial problem among spine surgeons. In this background,non- fusion or dynamic stabilization being proposed to preserve segment motion and prevent adjacent segment degeneration is developing rapidly. Dynamic neutralization system( Dynesys)is one of posterior pedicle fixation- based dynamic stabilization devices and has been used clinically for 20 years. It not only obtained encouraging clinical effect but also has some controversy. This review mainly summarize biomechanical study and clinical experience of Dynesys.
