集中多媒体法在膝关节置换患者术前访视中的运用

Application of centralized multimedia method in preoperative visit of patients with total knee replacement

目的探讨集中多媒体法在择期关节置换患者术前访视中的运用效果。方法选择2013年3—8月收治于上海交通大学附属第六人民医院拟行髋膝关节置换的患者400例,随机分为观察组和对照组各200例,分别于术前1 d护理访视。观察组集中应用多媒体演示访视内容,集中互动回答问题和心理干预。对照组以一对一的口头交谈方式告知相同内容。两组患者均于访视后及术后第2天进行焦虑评分、患者及家属满意度评分、床位医师满意度评分;记录访视前、术前、术中和术后的血压和心率;统计两组患者访视所用的人力成本。结果观察组的焦虑评分、术中心率及血压、患者及家属满意度评分、床位医师满意度评分和人力成本节约方面与对照组比较,差异有统计学意义(P<0.05),而在术前与术后的血压和心率方面比较差异无统计学意义。结论集中多媒体法可以更加有效地减轻患者围手术期的生理和心理应激反应,提高患者、家属和医师的满意度,节约人力成本,可操作性强,内容形式规范化,患者及家属参与积极,有利于提高术前访视效果。

Objective To investigate the application visit of patients with total knee replacement. Methods were planned to receive total knee replacement in S value of centralized multimedia method in preoperative From March to August 2013, a total of 400 patients who hanghai Sixth People＇s Hospital, Shanghai Jiao Tong University were randomly and equally divided into the experimental group and the control group. The experimental group received preoperative visit by multimedia presentation. The questions raised by patients were answered together. The patients also received group psychological intervention. The control group received preoperative visit by an oral introduction of the surgery. The anxiety scale and satisfaction scale were used. The blood pressure and heart rate were recorded. And the labor costs were analyzed. Results The anxiety score, blood pressure and heart rate during the surgery, satisfaction scores of patients and surgeons, and labor costs between the two groups had significant differences （ P 〈 0.05 ） , while the differences in blood pressure and heart rate between the two groups before and after operation were not significant. Conclusion The centralized multimedia method can effectively reduce the stress responses of patients during perioperation, improve the satisfaction of patients and surgeons, and save labor costs. The method has high operability with standardized contents and form, resulting in active involvement of patients and family members to improve the outcomes of preoperative visit
