摘要
目的探讨实时剪切波弹性成像应用于病理性肌紧张状态的可行性。方法以实时剪切波弹性成像技术检测30例临床诊断为颈肩肌筋膜疼痛综合征患者的斜方肌,并检测30例对照组的斜方肌降部肌腹,比较两组的组织弹性图和杨氏模量值。结果病例组斜方肌僵硬或触及痛性筋结或筋束的肌紧张处组织弹性图以绿蓝色为主,组织弹性图评分3.71±1.53,杨氏模量值(43.4±15.6)kPa;对照组组织弹性图以蓝色为主,组织弹性图评分1.60±0.72,杨氏模量值(24.0±7.6)kPa,两组比较差异均有统计学意义(P=0.000)。结论实时剪切波弹性成像是评价病理性非自主性肌紧张的一种可行的客观方法。
Objective To obtain the elasticity value of the trapezius in involuntary nervous and relaxation states with real-time shear wave elastography (SWE), in order to explore the feasibility of quantitative elastography with SWE in diagnosis of neck and shoulder myofascial pain syndrome. Methods SWE was performed in the descending part of the trapezius of 30 healthy volunteers and 30 cases who were diagnosed with neck and shoulder myofascial pain syndrome, and compared the tissue elasticity chart and Young's modulus between the two groups. Results The tissue elasticity chart of the case group was green- blue-based with the score was 3.71 ± 1.53 and the Young's modulus was (43.4 ±15.6)kPa, whereas the control group was mainly blue with the score was 1.60 ± 0.72 and the Young's modulus was (24.0 ± 7.6) kPa. The differences between two groups were statistically significant ( P = 0.000). Conclusions SWE can assess the trapezius involuntary nervous state by quantitatively measuring the elastic modulus, and it had definite clinical value in evaluating the trapezius involuntary nervous state.
