可塑性夹板中立位固定治疗伸直型桡骨远端骨折的临床研究 被引量:11

摘要 目的:观察比较可塑性夹板与石膏中立位固定治疗伸直型桡骨远端骨折的临床疗效。方法:将67例伸直型桡骨远端骨折患者,按照时间顺序随机分为治疗组和对照组。治疗组采用可塑性夹板中立位固定,对照组采用管型石膏外固定,两组分别在固定6周后去除外固定,观察周期3个月。各组分别于固定后即刻、固定后2周、固定6周后行患侧腕关节正侧位X线片,运用PACS系统分析测量各组X线片尺偏角、掌倾角、桡骨高;按照改良Green和O’Brien腕关节评分系统对患腕关节功能进行评价。结果:治疗组患者骨折复位及固定6周后尺偏角、掌倾角、桡骨高与对照组比较差异均讲学意义(P>0.05);治疗组临床疗效优于对照组(P<0.05),且治疗组临床并发症少于对照组。结论:可塑性夹板中立位固定治疗伸直型桡骨远端骨折疗效确切且并发症少。 Objective:To observe the effect of plastic splint neutral fixation on extension type of distal radius fractures.Methods:All 67 cases of extension type distal radius fracture patients were randomly divided into treatment group and control group.The treatment group was treated with the plastic splint neutral position,and the control group was treated with the traditional plaster external fixation.All of them removed the external fixation after 6 weeks,and were followed up for 3 months.All of the patients received the wrist X-ray immediately,2 weeks and 6 weeks after the fixation.The ulnar inclination,palmar tilt,and high of radius in X-ray were measured and analyzed by picture archiving and communication systems.The function of wrist joints was evaluated by modified Green and O’ Brien wrist score system.Results:The two groups had no significant difference(P〉0.05)on ulnar inclination,palmar tilt,and high of radius 6 weeks after the fixation.The clinical effect of treatment group was better than that of control group(P〈0.05),and the adverse events and complications in the treatment group were significantly less than those in the control group.Conclusion:The plastic splint neutral fixation has curative effect and less complications than the traditional plaster external fixation on extension type distal radius fracture.
