肺炎支原体感染患儿血清细胞因子及其IgE水平在哮喘发病中的相关性研究 被引量:17

摘要 目的研究儿童肺炎支原体(MP)感染与哮喘关系及血清细胞因子、IgE的检测在哮喘发病过程中的临床价值。方法 76例哮喘患儿设为哮喘组,89例肺炎患儿设为肺炎组,56例健康儿童设为对照组,比较哮喘组、肺炎组MP-Ab阳性率,并根据哮喘组MP-Ab检测结果分为MP阳性组(43例)及MP阴性组(33例),比较四组血清细胞因子、IgE检测情况及肺炎组、哮喘组MP阳性患者IgE与各细胞因子的相关性。结果哮喘组MP阳性率56.6%,高于肺炎组MP-Ab阳性率21.3%,差异有统计学意义(P<0.05);IL-4、IL-13、IgE浓度水平在对照组、肺炎组、哮喘MP阴性组及哮喘MP阳性组中依次升高,IFN-γ浓度水平哮喘MP阳性组均低于其他各组,TNF-a哮喘MP阳性组均高于于其他各组,组间差异有统计学意义(P<0.05);肺炎组、哮喘组MP阳性患者中IgE与IL-4、IL-13及TNF-a均呈正相关,与IFN-γ呈负相关,组间均有统计学意义(P<0.05)。结论 MP在哮喘的发作中既为感染源也为变应原,相关细胞因子与IgE的关联性变化在哮喘的发病过程起重要的作用。 Objective To study the the relationship between serum cytokines and IgE levels in asthma in children with Mycoplasma pneumoniae infection. Methods 76 children with asthma were set as asthma group, 89 cases of pneumonia were set to the pneumonia group and 56 healthy children were set as control group. MP-Ab positive rate, serum cytokine and IgE were observed and analyzed. Results MP positive rate of Asthma group (56.6 %) was higher than that of the pneumonia group (21.3 %) (P〈0.05). Concentration levels of IL-4, IL-13 and IgE were increased in the control group, pneumonia group, MP negative in asthma group and MP positive in asthma group. Concentration levels of IFN-γ in MP positive in asthma group were lower than that of other groups. TNF-a in MP positive group were higher than in the other groups (P〈0.05). The positive MP IgE of pneumonia group and asthma group were positively correlated with IL-4, IL-13 and TNF (P〈0.05). Conclusion MP in asthma is the source of infection for allergen IgE play an important role in the pathogenesis of asthma.
