艾灸联合五藤汤加减治疗活动期强直性脊柱炎

Treatment on ankylosing spondylitis at active stage with combination moxibustion and modified Wuteng Decoction

目的:观察艾灸联合五藤汤加减治疗活动期强直性脊柱炎(AS)的疗效和安全性。方法:选取42例符合AS活动期诊断的患者,予艾灸和五藤汤治疗。艾灸选取督脉和足太阳膀胱经腧穴及经外夹脊穴等穴位,每周3次。五藤汤水煎服,每日1剂。连续治疗1个月为1个疗程。分别观察其临床指标、实验室指标、安全性指标,连续治疗3个疗程后,判断其疗效。结果:1临床缓解8例,显效21例,有效10例,无效3例,总体有效率为92.86%,治疗后患者的病情活动指数(BASDAI)、功能指数(BASFI)、胸廓活动度、Schober试验、指-地距、枕-墙距、红细胞沉降速率(ESR)、C-反应蛋白(CRP)均较治疗前有显著改善(P<0.05,P<0.01);2患者未见明显不良反应。结论:艾灸联合五藤汤加减治疗活动期AS能改善患者的BASDAI、BASFI、胸廓活动度、Schober试验、指-地距、枕-墙距离、ESR、CRP等指标,治疗有效率高,且不良反应少,安全性较好,值得临床推广。

Objective： To observe the clinical efficacy and safety of combination moxibustion and modified Wuteng Decoction in the treatment of patients with ankylosing spondylitis at active stage. Methods： Forty-two patients with ankylosing spondylitis at active stage were included and treated with combination moxibustion and Wuteng Decoction. Moxihustion points included points in governor vessel, Taiyang bladder meridian of foot and Jiaji points. Patients were treated with moxibustion 3 times per week and Wuteng Decoction 1 dose a day. A month as a treatment course, the treatment lasted for 3 treatment courses. Clinical indicators, laboratory indexs, safety indexs, and the clinical curative effect were observed. Results： （1）Eight cases of patients were relieved, 21 patients had achieved significant results, 10 patients were effective, and 3 patients were invalid. The total effective rate was 92.86%. The BASDAI, BASFI, thoracic mobility, Schober experiment, finger-floor distance, occiput- wall distance, ESR and CRP were improved significantly compared with treatment before （P〈0.05, P〈0.01）. （2）There were not significant adverse reactions in patients. Conclusion： Combination moxibustion and modified Wuteng Decoction could improve BASDAI, BASFI, thoracic mobility, Schober experiment, finger-floor Distance, occiput-wall distance, ESR and CRP of patients, and with higher effective rate, fewer adverse reaction, better security, which is worthy of clinical promotion.
