原发性骨质疏松症血瘀与骨代谢关系研究

Research on the relationship between blood stasis and bone metabolism with primary osteoporosis

目的:通过检测女性原发性骨质疏松症(POP)"肾虚血瘀"证患者不同年龄段血细胞参数及骨代谢标志物水平,探讨血瘀与骨代谢相关性。方法:64例符合纳入标准的患者按每10岁分为1组,共分3组,检测所有患者血细胞参数[平均红细胞体积(MCV)、红细胞体积分配宽度(RDW-cv)、平均血小板体积(MPV)、血小板体积分配宽度(PDW)]、骨代谢标志物(P1NP、β-CTx)、骨密度(BMD),将患者年龄、血细胞参数、BMD、骨代谢标志物进行Pearson相关分析。结果:患者血瘀指标与年龄、BMD和P1NP均有显著相关性:MCV与年龄呈低度正相关,r=0.257,P<0.05;PDW与BMD呈低度负相关,r=-0.274,P<0.05;RDW-cv与P1NP呈中度正相关,r=0.431,P<0.01。结论:POP患者血瘀引起骨代谢异常,骨转换和骨量丢失加快,发生骨质疏松。血瘀与骨代谢关系密切,是骨质疏松发病的重要病理基础。

Objective： To explore the correlation between blood stasis and bone metabolism by detecting the parameters of blood cells and bone metabolic markers of female primary osteoporosis patients with syndrome of kidney deficiency and blood stasis at different ages. Methods： Sixty-four patients meeting the inclusion criteria were divided into three groups by every 10 years of age. The parameters of blood cells： mean corpucular volume （MCV）, red blood cell distribution width-coefficient of vaviation （RDW-cv）, mean platelet volume （MPV）, platelet distribution width （PDW）, bone metabolic markers （P1NP, β -CTx）, bone mineral density （BMD） were tested. Pearson correlation analysis was used to evaluate the relationship between blood cells parameters and patient＇s age, BMD and bone metabolic markers. Results： The results showed that parameters of blood ceils had significant linear correlation with the patient＇s age, BMD and P1NP. The detailed results were as follows： There was a low positive correlation between MCV and age, r=0.257, P〈0.05; a low negative correlation between PDW and BMD, r=0.274, P〈0.05; a middle positive correlation between RDW-cv and P1NP, r=0.431, P〈0.01. Conclusion： Blood stasis causes abnormal bone metabolism, which leads to the acceleration of bone turnover and bone mineral loss in patients with primary osteoporosis. Blood stasis and bone metabolism are closely related. Blood stasis is an important pathologic basis of osteoporosis.
