切开与闭合复位交锁髓内钉固定治疗56例股骨骨折效果分析

Effect comparison of open reduction and closed reduction with intramedullary nail fixation for 56 cases with femoral fracture

目的探讨在股骨骨折患者中,对比切开与闭合复位交锁髓内钉固定治疗方案的临床疗效。方法选取我院收治的股骨骨折患者56例进行分析,将其随机分为对照组和观察组。对照组采用闭合复位交锁髓内钉固定方案,观察组采用切开复位交锁髓内钉固定方案。观察两组患者的临床治疗效果,以及并发症发生情况。结果在术中出血量、住院时间指标上,组间相比差异不明显,不具有统计学意义(P>0.05)。观察组患者的手术时间明显短于对照组,患者在出血、感染、骨不连、伤肢短缩等并发症的发生率(7.1%)低于对照组(28.6%),术后骨痂的生长速度更快,和对照组相比差异明显,具有统计学意义(P<0.05)。结论在股骨骨折患者中,应用切开复位交锁髓内钉固定治疗方案,能够缩短手术时间,减少并发症的发生,加快术后骨痂的生长,有利于患者的预后和恢复,值得临床推广。

Objective To investigate and compare open reduction and closed reduction with intramedullary nall fixation for patients with femoral fracture. Methods 56 cases of patients with femoral fractures were selected and analyzed, and randomly divided into a control group and an observation group. The control group received closed reduction and interlocking intramedullary nall fixed scheme, the observation group received fixed scheme of open v reduction and interlocking intramedullary nall. The clinical therapeutic effect of the two groups were observed. Results The amount of bleeding, hospitalization time difference was not significant, without statistically significant （P 〉 0.05）. Patients in the observation group operation time was shorter than that in the control group, bleeding, infection, nonunion, the injured limb shortening complications was （7.1%）, while that in the control group was（28.6%）, postoperative callus growth was faster, with statistical significance （P 〈 0.05）. Conclusion The patients with femoral fractures, using open reduction and interlocking intramedullary nall fixation can shorten operation time, reduce occurrence of complications, accelerate the growth of the callus after surgery, is worth the clinical promotion.
