摘要
【目的】了解痛风患者对饮食控制知识的掌握情况,为针对性的健康教育提供依据。【方法】现场问卷调查175名原发性痛风患者连续病例,内容包括患者的人口学资料、病情评估指标及关于饮食控制的12道选择题。【结果】共收集150份有效问卷,所有患者正确作答的题数为4.3±1.5。将正确作答的题数≥6定义为知晓,所有患者的知晓率为22.0%。3/4的患者了解海鲜、内脏及饮酒不利于痛风病情控制,66.0%的患者了解红肉会升高血尿酸,仅8.7%的患者了解低脂奶制品可降低血尿酸,40.7%的患者仍认为嘌呤含量高的蔬菜对痛风不利,76.0%的患者仍认为豆制品会升高血尿酸,仅14.7%、12.7%及40.0%的患者分别了解富含果糖的水果、含糖饮料及果汁不利于痛风病情控制。Logistic回归分析提示性别、痛风急性发作的频率是知晓的预测因素。【结论】相当部分的痛风患者所掌握的饮食控制知识过于陈旧,尤其是不了解一些新的危险及保护因素,今后应加强针对更新知识的患者教育,尤其是痛风反复发作的男性患者。
【Objective】 To investigate gout-related diet control knowledge of patients and identify the target of health education for patients with gout. 【Methods】 175 continuous patients with primary gout were interviewed with self-designed questionnaire including patient's demographic data, the assessment of disease burden and 12 questions about diet control knowledge. 【Results】 150 valid questionnaires were collected. The mean of all patients' correctly answered items was 4.3 ± 1.5. The participants were considered having diet control knowledge if he or she correctly answered ≥6 items and the rate of having knowledge for all patients was 22.0%.Three fourths of patients were well informed about the hazard of seafood, organ meat or alcohol for gout control and 66.0% knew that red-meat will increase serum uric acid(s UA). Only 8.7% of patients knew that low-fat dairy products could lower s UA, and 40.7%still thought purine-rich vegetables were bad for gout patients, 76.0% thought that soy products could increase s UA. There were14.7%, 12.7% and 40.0% of patients who were aware that fruits rich in fructose, sugar-sweetened soft drinks and fruit juices were unfavorable to gout control, respectively. Logistic regression revealed that gender and acute attack frequency significantly predicted that patients had diet control knowledge. 【Conclusion】 Gout patients' knowledge of gout-related diet control was out-of-date,especially about some new risk factors and protective factors. Patients ' education in future should be emphasized on the update of gout diet control knowledge, especially for male patients and patients suffered repeated gout attack.
