2型糖尿病患者踝肱脉搏波传导速度与尿蛋白的相关性 被引量:3

摘要 目的尿蛋白与糖尿病大血管病变的发生与发展密切相关。文中探讨在2型糖尿病患者中,踝肱脉搏波传导速度(brachial-ankle pulse wave velocity,ba PWV)与尿蛋白之间的相关性。方法 142名2型糖尿病患者按照24 h尿白蛋白水平分为正常对照组[24 h尿白蛋白定量<30 mg,n=84]、微量蛋白尿组(30 mg≤24 h尿白蛋白定量<300 mg,n=26)、临床蛋白尿组(24 h尿白蛋白定量≥300 mg,n=33),所有2型糖尿病患者均检测空腹血糖、血脂、肾功能、糖化血红蛋白、胰岛功能、C反应蛋白、血压、计算体重指数,并通过自动脉冲式方法测量其ba PWV,并采用Pearson相关系数分析ba PWV与其他因素的相关性及多元回归分析ba PWV的相关因素。结果微量蛋白尿组及临床蛋白尿组的ba PWV分别为(1649.19±229.36)cm/s、(1759.21±291.05)cm/s,与对照组[(1522.33±248.37)cm/s]比较均显著增快(P<0.05)。Pearson相关分析示24 h尿白蛋白与ba PWV呈正相关(r=0.347,P<0.01)。多元线性回归示ba PWV与24 h尿白蛋白具有相关性(P<0.05)。结论 2型糖尿病患者尿蛋白与ba PWV关系密切。 Objective Urinary protein is closely related to the development and progression of major vascular lesionsin diabetic patients.This study was to investigate the correlation between brachial-ankle pulse wave velocity(baPWV) and urinary protein in patients with type 2 diabetes mellitus ( T2DM) . Methods According to the level of 24-hour urinary albumin ( UAL) , 142 T2DM patients were divided into three groups:control (UA330 mg, n=84), microalbuminuria(UAL3300 mg, n=26), and clinical albuminuria group ( UAL≥300 mg, n=33) .All the patients underwent examination offasting blood glucose, blood lipid, renal function, HbA1c, islet func-tion, C-reactive protein, blood pressure, and BMI.Their baPWV was determined with an Omron atherosclerosis detector and the correla-tion of baPWV with other factors evaluated by Pearson analysis and multivariate linear regression analysis. Results The baPWVwas significantly higher in themicroalbuminuria([1649.19±229.36]cm/s) and clinical albuminuria ([1759.21±291.05]cm/s) than in the control group ([1522.33±248.37]cm/s) (P30.05).Pearson analysis showed a positive correlation between 24-hour UAL and baPWV ( r=0.347, P30.01) and multivariate linear regression analysis manifested the same correlation ( P30.05) . Conclusion The level of urinary protein is closely correlated with baPWV in T2DM patients.
