脑电熵指数和双频谱指数用于小儿七氟烷麻醉深度监测对比 被引量:7

Comparison between EEG entropy and bispectral index in monitoring pediatric sevoflurane anesthesia depth

摘要 目的比较脑电熵指数以及双频谱指数在小儿七氟炕麻醉深度监测中的效果。方法选择80例行择期手术的小儿患者作为研究对象,监测其脑电熵指数和双频谱指数,比较状态熵(SE)、反应熵(RE)以及脑电双频指数(BIS)的变化情况。结果意识消失、插管即刻、插管后20S、1min以及3rain等时刻,BIS值与插管前时刻比较差异均未见统计学意义(P〉0.05),意识消失、插管前以及插管后1rain等时刻与基础值比较差异有统计学意义(P〈0.05)。插管后各时刻的SE值与插管前比较差异有统计学意义(P〈0.05),而意识消失以及插管前与SE基础值比较差异有统计学意义(P〈0.05)。插管后1min以及3min的RE值与插管前比较差异有统计学意义(P〈0.05),意识消失以及插管前与RE基础值比较差异有统计学意义(P〈0.05)。结论脑电熵指数在小儿七氟烷麻醉深度监测中的应用与双频谱指数相比效果更好,可更加及时地反映患儿意识的变化过程,敏感性较高,量化表示患儿的镇静深度,评估作用更强,控制患儿手术过程中的麻醉情况,确保手术的顺利进行。 Objective To compare the effects of EEG entropy and bispectral index on momtonng pediatric sevoflurane anesthesia depth. Methods Eighty pediatric patients with elective surgical procedures were chose as the research objects, the EEG entropy and bispectral index, were monitored, SE, RE and the change of the BIS were compared. Results At consciousness disappear moment, immediate intubation and after intubation for 20 seconds, 1 minute and 3 minutes, the BIS values had no significant differences compared with the moment before intubation(P 〉 0.05 ). The BIS values had significant differences in basic value compared with consciousness disappear moment, before intubation and 1 minutes after intubation time (P 〈 0.05 ). In each moment after intubation SE, SE values had significant differences com- pared with before intubation (P 〈 0.05 ), and also had significant differences before unconsciousness moment and intubation compared with the basic value (P 〈 0.05 ). One minute and 3 minutes after intubation, RE values had significant differences compared with before intubation (P 〈 0.05 ), and also had significant differences before unconsciousness moment and intubation compared with the basic value (P 〈 0.05). Conclusions The application effect of EEG entropy index on monitoring the pediatric sevoflurane anesthesia depth is better compared with bispectral index, it can reflect changes in the children with consciousness process more timely, with higher sensitivity, reflect children with sedation depth quantitatively, evaluation function is stronger,it also can control the anesthesia in process of operation,to ensure the operation smoothly.
