阿德福韦酯联合黄芪颗粒治疗慢性乙型肝炎的临床疗效及对患者血清ALT和总胆红素的影响

Clinical Efficacy of Adefovir Dipiroxil Combined with Astragalus Granule on the Treatment of Chronic Hepatitis B and its Influence on the Serum ALT and Total Bilirubin

目的:探讨阿德福韦酯联合黄芪颗粒治疗慢性乙型肝炎的临床疗效及对患者血清ALT和总胆红素水平的影响。方法:80例慢性乙型肝炎患者随机分为观察组和对照组各40例。对照组给予阿德福韦酯治疗,观察组在对照组基础上加用黄芪颗粒治疗。两组疗程均为24周。疗程结束后比较两组患者的临床疗效、血清丙氨酸转氨酶(ALT)与总胆红素水平变化、HBe Ag转阴率、HBV-DNA转阴率及药品不良反应发生情况。结果:观察组治疗后总有效率显著高于对照组(P<0.05);治疗后,两组患者血清ALT、总胆红素水平均较治疗前显著降低(P<0.05),且治疗后观察组血清ALT、总胆红素水平显著低于对照组(P<0.05);观察组HBe Ag转阴率显著高于对照组(P<0.05),HBV-DNA转阴率略高于对照组,但差异无统计学意义意义(P>0.05);两组药品不良反应发生率比较,差异无统计学意义(P>0.05)。结论:阿德福韦酯联合黄芪颗粒治疗慢性乙型肝炎的临床疗效显著,能显著降低患者血清ALT和总胆红素水平,提高HBe Ag转阴率,且不良反应少,值得临床推广。

Objective： To investigate the clinical efficacy of adefovir dipivoxil combined with astragalus granule therapy of chronic hepatitis B and its effect on the serum ALT and total bilirubin（ TB） level of patients. Methods： Eighty patients with chronic hepatitis B patients were randomly divided into observation group and control group,40 cases each.The control group received adefovir dipivoxil capsule,and observation group combined with astragalus granules on the basis of treatment in the control group. The patients of two groups were treated for 24 weeks. By comparison,the clinical efficacy of the two groups of patients,changes in the level of serum ALT and TB of the two groups,negative seroconversion rate of HBe Ag,negative conversion rate of HBV-DNA and adverse drug reactions during treatment were observed. Results： The total efficiency of observation group after treatment was significantly higher than the control group,and had a statistically significant difference（ P〈0. 05）. After treatment,the serum ALT,TB level of two groups was significantly lower than before treatment（ P〈0. 05）,and those levels of observation group were significantly lower than the control group（ P〈0. 05）.The negative seroconversion rate of HBe Ag in observation group was significantly higher than the control group,and had significant difference（ P〈0. 05）. The negative conversion rate of HBV-DNA of observation group was slightly higher than the control group,but had no statistical significance（ P〉0. 05）; the incidence of adverse reactions in two groups had no statistically significant difference（ P〉0. 05）. Conclusion： The clinical efficacy of adefovir dipivoxil combined with astragalus granules is obvious,the therapy could descend ALT and total bilirubin level,increase negative seroconversion rate of HBe Ag. Clinical application is safe and reliable,it has important clinical research value.
