我院门诊处方点评与分析

Analysis and evaluation of prescriptions in outpatient departments

目的分析门诊处方不合理使用情况,促进临床合理用药。方法采用回顾分析方法,抽查2013年7月至2014年6月门诊处方,每月随机抽取200张,共计2 400张。根据卫生部《处方管理办法》、《医院处方点评管理规范(试行)》的要求,进行分析。并对使用上海大通临床用药决策支持软件后的改观情况进行评价。结果使用该软件前,门诊电子处方合理率为88.25%;使用该软件后,门诊电子处方合理率为93.5%,处方合理率显著提高。其中抗菌药物的处方量减少,应用抗菌药物的科室分布以妇科、呼吸科、耳鼻咽喉科、泌尿外科为最高。结论使用该用药决策支持软件可以提高处方合理率,但仍需不断提高药师审方能力,完善医院的处方集,改善该软件的用药信息,使其不断更新等。

Objective To analyze the drug use of outpatient prescriptions ,and to promote the rational drug use in clinic .Methods A retrospective analysis was carried out on prescriptions of outpatient department ,collected from our hospital from July 2013 to June 2014 ,200 per month .A total of 2 400 outpatient prescriptions were collected randomly .According to the Ministry of Health Pre‐scription Management Methods ,and the Management Criteria of Hospital Prescription Comment （trial） ,comparative analysis was performed .The improving situation after using Shanghai Datong CMDS was evaluated .Results Before using the software ,the ra‐tional rate of outpatient prescriptions was 88 .25% ,while after using the software ,the rational rate of outpatient prescriptions was 93 .5% ,which increased significantly .Prescriptions of antibacterial agents declined ,while the use of antimicrobial agents in the department of gynecology ,respiratory ,otolaryngology ,and urology was the highest .Conclusion Although CMDS can improve the rational rate of the outpatient prescriptions ,the ability of pharmacists ,the hospital′s formulary ,and medication constantly updated information about the software and other issues still need to continue to improve .
