随机尿香草扁桃酸与肌酐比值检测对神经母细胞瘤诊断价值的Meta分析 被引量:4

Meta analysis of the value of VMA/Cr at random urine collection in the diagnosis of neuroblastoma

摘要 目的评估随机尿香草扁桃酸与肌酐比值(VMA/Cr)对神经母细胞瘤诊断的价值。方法通过对MEDLINE、EMbase、中国期刊全文数据库、中国科技期刊全文数据库、万方数据库进行系统检索,检索起止时间为建库之日至2014年7月收录的所有已正式发表的科研文献,英文检索词包括:vanilmandelic acid,creatinine,VMA,neuroblastoma,NB;中文检索词包括:"香草扁桃酸"或"VMA"、"肌酐"、"神经母细胞瘤"或"NB"。结果神经母细胞瘤组与健康对照组随机VMA/Cr水平有统计学意义,神经母细胞瘤组随机尿VMA/Cr水平和阳性率显著高于健康对照组。随机尿VMA/Cr用于诊断神经母细胞瘤的汇总敏感度为0.84(95%CI:0.80-0.88),汇总特异度为0.93(95%CI:0.92-0.95),SROC AUC为0.9458。结论随机VMA/Cr用于诊断神经母细胞瘤的敏感度和特异度都较高,具有较高的诊断应用价值。 Objective To assess the value of vanilmandelic acid/creatinine(VMA/Cr)at random urine collection in the diagnosis of neuroblastoma. Methods CNKI,VIP Information and Wanfang data were searched for studies on the vanilmandelic acid/creatinine and neuroblastoma.Reference lists were reviewed to seek the data missed by this search.Fixed and random effects meta-analysis was used to pool data. Results The level of VMA/Cr at random urine collection was significantly higher in the neuroblastoma patients than in the control group. The pooled sensitivity and pooled specificity of the VMA/Cr in the diagnosis of neuroblastoma were 0.84(95% CI:0.80-0.88)and 0.93(95% CI:0.92-0.95)respectively,and the SROC AUC was 0.9458. Conclusion The sensitivity and specificity on VMA/Cr at random urine collection in the diagnosis of neuroblastoma at both at a high level. Thus,VMA/Cr is a good biomarker in diagnosing neuroblastoma.
