2012-2013年某院国家基本药物使用情况分析

Analysis of the Utilization of National Essential Medicines in a Hospital from 2012 to 2013

目的:为促进国家基本药物使用和临床合理用药提供参考。方法:利用医院信息系统中的"药品综合查询"程序,采用金额排序法,对某院2012-2013年国家基本药物使用数据进行统计、分析。结果:该院2012-2013年在用国家基本药物分别为262、253种,国家基本药物品种数覆盖率分别为50.38%、48.65%,使用金额占全院全年用药总金额的比例分别为13.03%、11.38%。2012-2013年,该院中成药类国家基本药物使用金额排序分别为第2、1位,调节水、电解质及酸碱平衡药类国家基本药物排序分别为第1、2位,激素及影响内分泌药类国家基本药物排序分别为第4、3位,心血管系统用药类国家基本药物排序也较靠前;2013年抗微生物药类国家基本药物使用金额排序大幅下降。中成药类国家基本药物中内科用药使用金额构成比很高。结论:该院国家基本药物的覆盖率、使用率离卫生部门的目标还有差距,一些价格低的国家基本药物在该院并没有使用。建议该院根据《国家基本药物目录(2012版)》要求增加一些国家基本药物品种,特别是中成药品种;临床应首先考虑使用国家基本药物;医保中心应考虑增加基本药物目录中一些药物品种为医保用药。

OBJECTIVE：To provide reference for promoting the utilization of national essential medicines and rational drugs use in the clinic.METHODS：Using the＂Drug comprehensive inquiry＂program in hospital information system,the data about the utilization of national essential medicines in a hospital from 2012 to 2013 was statistically analyzed by amount ordering.RESULTS：There were respectively 262 and 253 kinds of national essential medicines in the hospital from 2012 to 2013 with the varieties coverage rate of 50.38% and 48.65%,respectively;the proportion of utilization amount in the annual total amount in hospital was respectively 13.03% and 11.38%.In 2012 and 2013,the utilization amount ordering of Chinese patent medicines respectively stood the second and first one;the national essential medicines ordering of regulation of water,electrolyte and acid-base balance was respectively the top one and two;the hormones and endocrine medicines was respectively the forth and third one;and cardiovascular medicines was also top-ranking.The utilization amount of microbiological medicines was significantly declined in 2013.Among the Chinese patent medicines in national essential medicines,the utilization amount constituent ratio in internal medicines department was high.CONCLUSIONS：There is a gap between the coverage rate and utilization rate of the national essential medicines in the hospital and the goal of health sectors and some national essential medicines with low price is not utilized in the hospital.It is suggested to add some national essential medicines varieties based on the requirements of National Essential Medicines List（2012 edition）,especially the Chinese patent medicines varieties;the clinic should forstly consider the utilization of national essential medicines;health insurance center should consider adding some medicines varieties in national essential medicines list as medicare medicines.
