冠状动脉疾病患者CD36基因多态性与心血管相关因素的关系 被引量:4

摘要 目的研究CD36基因多态性与冠状动脉疾病(CAD)危险因素之间的关系。方法选择解放军空军总医院<60岁90例CAD患者,抽取患者空腹静脉血检测血红蛋白(Hb)、白细胞(WBC)、单核细胞(M)、嗜碱性粒细胞(B)、空腹血糖(FPG)、高敏C反应蛋白(hs-CRP)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)及三酰甘油(TG)、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)和脂蛋白(α)[(Lp(α)]。同时记录患者的体质量、收缩压、舒张压,并计算体质量指数和平均动脉压。利用变性高效液相色谱技术(DHPLC)分析包含内含子片段的外显子4和5的扩增产物,分析基因型和各变量之间的相关性。结果发现CD36基因有两种多态性变异:IVS3-6 T/C(rs3173798)和IVS4-10 G/A(rs3211892)。其中IVS3-6 T/C多态性的C等位基因与肥胖、糖尿病、血清Lp(α)浓度降低以及心肌梗死发病年龄提前有关。而IVS4-10 G/A多态性的A等位基因与心肌梗死发病较晚和白细胞数量增加有关。结论 CD36的IVS3-6 C等位基因与心血管疾病的危险因素(如hs-CRP、BMI和2型糖尿病)有关,与心肌梗死发病年龄提前有关;而IVS4-10 A等位基因与心肌梗死发病年龄推迟有关。CD36的多态性在CAD中的作用仍需进一步研究。 Objective To investigate the relationship between CD36 gene polymorphism and coronary artery disease ( CAD) ’ s risk factors.Methods 90 CAD patients aged〈60 were collected, hemoglobin ( Hb) , white blood cell ( WBC) , mononuclear cells ( M) , basophils ( B) ,fasting blood glucose ( FPG) , high sensitive C reactive protein ( hs-CRP) , total cho-lesterol ( TC) , high density lipoprotein cholesterol ( HDL-C) and low density lipoprotein cholesterol ( LDL-C) and triglyceride (TG),apolipoprotein A1(ApoA1), ApoB and lipoprotein (α) [Lp (α)]were detected. At the same time recording quality, systolic pressure, diastolic pressure volume of patients, and calculated the body mass index and mean arterial pressure.Using denaturing high performance liquid chromatography to analyze intron fragment containing exon 4 and exon 5’ s amplicon, ana-lyzed the correlation between the genotype and the variables.Results There are two kinds of polymorphism of CD36 gene:IVS3-6 T/C (rs3173798) and IVS4-10 G/A (rs3211892).The IVS3-6 TC/polymorphism of C alleles was correlated with o -besity,diabetes, serum Lp ( a) concentration decreases and the early age onset of myocardial infarction .While the number of IV4S-10 G/A polymorphism of A alleles was related to late myocardial infarction and increase of white blood cell .Conclusion CD36’ s IVS3-6 C allele was correlated with the risk factors of cardiovascular disease ( such as hs-CRP, BMI and type 2 di-abetes mellitus) , and correlated with early age onset of myocardial infarction;while the IVS4-10 A allele was correlated with late age onset of myocardial infarction.The role of CD36 polymorphism in CAD is still needs further research.
