摘要
目的探讨临床药师参与急性脑梗死患者营养治疗过程中的药学服务模式。方法通过参与1例急性脑梗死患者营养治疗的案例,临床药师综合评估了患者的营养风险,协助治疗组制定了个体化营养治疗方案,监护患者营养治疗全过程,结合国内外营养治疗指南及文献为治疗组提供建设性的药学建议。结果患者在营养治疗过程中未出现严重并发症,对营养方案耐受较好。结论临床药师参与患者营养治疗全过程,促进了营养药物安全、合理利用,减少了营养治疗的并发症,有利于患者疾病治疗及预后。
Objective To investigate pharmaceutical care mode of clinical pharmacists in nutrition treatment for patients with acute cerebral infarction. Methods Clinical pharmacists were involved in the nutrition treatment of 1 patient with acute cerebral infarction. The clinical pharmacists evaluated the nutritional risk of the patient, assisted the treatment team to modify and monitor individualized nutrition treatment, and offered constructive advice for the guidelines of nutrition treatment. Results Without severe adverse effects, the patient tolerated the nutrition treatment. Conclusion By participating in the nutrition treatment, clinical pharmacists facilitate the rational use of nutraceuticals to help disease treatment and prognosis.
