子宫内膜癌患者血清CA125检测的临床意义

Clinical significance of serum CA125 detection in patients with endometrial cancer

目的探讨子宫内膜癌患者血清CA125检测的临床意义。方法选取2004年1月至2013年12月期间在四川省中江县人民医院妇产科住院的子宫内膜癌患者151例,收集患者的临床病理资料,并采集患者术前、术后辅助治疗前、复发患者确诊后的空腹外周静脉血,测定血清CA125水平。分析CA125与临床病理资料的关系和复发患者复发前后CA125水平变化。结果 151例子宫内膜癌患者中CA125阳性患者43例(28.48%)。CA125阳性率与临床分期、肿瘤直径≥2cm、病理分级、肌肉浸润≥1/2、宫颈受累、附件转移、腹腔细胞学阳性、淋巴结转移显著相关(x^2值分别为21.937、11.994、14.728、7.560、22.490、11.421、20.001、8.687,均P<0.05),但与组织类型无显著相关性(x^2=1.575,P>0.05)。15例子宫内膜癌复发患者术前CA125水平明显高于未复发患者,差异具有统计学意义(t=14.083,P<0.05)。结论子宫内膜癌患者术前血清CA125的检测有助于了解肿瘤的分期及侵犯范围,术后定期复查CA125水平有助于监测子宫内膜癌的病情进展。

Objective To explore the clinical significance of serum carbohydrate antigen-125 （ CA125 ） detection in patients with endometrial cancer.Methods A total of 151 patients with endometrial cancer were treated in obstetrics and gynecology department of People’s Hospital of Zhongjiang County during the period of January 2004 to December 2013.Clinical and pathological data of them were collected and their serum CA125 levels were measured using fasting peripheral venous blood before and after operation, and after diagnosis of recurrence, so as to analyze the correlation between serum CA125 levels and the clinical and pathological data as well as study the changes of serum CA125 levels before and after recurrence.Results CA125 positive rate was 28.48%（43/151）, which was significantly correlated with clinical tumor stage, diameter of tumor≥2cm, pathological grading, myometrium invasion≥1/2, cervical involvement, adnexa metastasis, positive peritoneal cytology and lymph node involvement （χ^2 value was 21.937, 11.994, 14.728, 7.560, 22.490, 11.421, 20.001 and 8.687, respectively, all P〈0.05）.There was no significant relationship between CA125 positive rate and tissue types （χ^2 =1.575,P〉0.05）.The preoperative CA125 levels of patients with endometrial carcinoma recurrence were significantly higher than those in patients without recurrence, and there was significant difference （t=14.083,P〈0.05）.Conclusion Detecting serum CA125 level before operation is valuable in predicting endometrial carcinoma stage and invasion, and regularly detection of CA125 level after operation may be helpful in monitoring the prognosis of endometrial cancer.
