摘要
目的观察小牛血去蛋白提取物对糖尿病肾病患者24h尿蛋白定量的影响。方法采用单盲法,将118例24h尿蛋白定量升高的糖尿病肾病患者随机分为观察组(38例)和对照组(80例),两组患者均常规降糖治疗,疗程均为2周,观察组加用注射用小牛血去蛋白提取物800mg静脉滴注,1次/d,对照组加安慰剂O.9%氯化钠注射液250ml,静脉滴注。观察两组患者治疗前后24h尿蛋白定量变化及不良反应。结果两组治疗前24h尿蛋白定量比较差异无统计学意义(z=1.100,P=0.270);观察组治疗后24h尿蛋白定量明显降低,与对照组比较,差异有统计学意义(z=-5.373,P=0.000);对照组治疗前、后24h尿蛋白定量比较,差异无统计学意义(z=0.375,P=0.707);两组患者在治疗期间均未出现不良反应。结论小牛血去蛋白提取物具有降低糖尿病肾病患者24h尿蛋白定量的作用,安全有效,值得临床推广。
Objective To observe effect of 24-hour urinary protein excretion with deproteinized calf blood protein extractives treatment in diabetic nephropathy patients. Methods Using single-blind method, 118 diabetic nephropathy patients with high 24-hours urinary protein excretion randomly assigned into control group ( 80cases ) and treatment group ( 38 cases ) ; Two groups of patients had routine hypoglycemic treatment for two weeks;in treatment group, they were combined with the deproteinized calf blood extractives ( 800mg a day, intravenous ) , the control group were only treated with placebo of 0.9% saline 250ml, intravenous infusion;the patients were observed 24-hour of urinary protein excretion and adverse reactions before and after treatment. Results Before treatment, 24-hour urinary protein was no significant difference in the two groups ( Z=-1.100, P=0.270 ) ;After the treatment, 24-hour urinary protein excretion in the treatment group was significantly lower ( Z=-5.373, P=0.000 ) , which was not changed significantly in the control group ( Z=-0.375, P=0.707 ) ;two groups of patients had no adverse reactions during treatment.Conclusion Deproteinized calf blood extractives has the role of reducing 24-hour urinary protein in diabetic nephropathy patients, and it is safe and effective, worthy of clinical application.
