高血压患者肱动脉压与中心动脉压的变化趋势及相关性研究 被引量:2

The study of change tendency and change correlation of brachial artery pressure and central arterial pressure in patients with hypertension

摘要 目的探讨高血压患者肱动脉压(b BP)与中心动脉压(CABP)随年龄增长的变化趋势及其可能的影响因素与机制。方法随机选取1 300例志愿者,年龄(54±24)岁,其中男性672例,女性628例,测量患者的中心动脉压、肱动脉压,根据年龄分为9组,分析收缩压、舒张压、脉压分别随年龄增长的变化趋势,计算2种测量方法的一致性及灵敏性。结果肱动脉压(b BP)与中心动脉压(CABP)随年龄增长的变化趋势均一致,其中舒张压(DBP)一致性较好,中心动脉压较肱动脉压敏感;肱动脉收缩压(b SBP)大于中心动脉收缩压(c SBP),差值为(15.15±10.20)mm Hg,肱动脉舒张压(b DBP)小于中心动脉舒张压(c DBP)(P<0.01),差值为(1.68±5.99)mm Hg,肱动脉脉压(b PP)大于中心动脉脉压(c PP)(P<0.01),差值为(16.94±10.67)mm Hg;多元逐步回归分析结果表明,c BP、b PP的主要影响因素均是年龄、心率、身高。结论肱动脉与中心动脉随年龄增长的变化趋势一致,大致呈上升趋势,其中DBP一致性较好,但2种测量方法所测得血压有差异,中心动脉压较肱动脉压敏感;中心动脉压、肱动脉压均与身高、心率、年龄有相关性。 Objective To study the center arterial systolic and brachial arterial pressure and pulse pressure change tendency along with the age between the two groups of hypertensive patients,and to discuss the affecting factors and its mechanism.Methods The hypertensive patients(n = 1300,including male 672,female 628,and aged 54 ±24) were selected randomly.The central arterial pressure and brachial arterial pressure were examined respectively.The patients were divided into nine groups according to their ages and the change tendency of the pressure with ages was analyzed.The consistency and sensitivity of two measuring methods were examined.Results The change tendency in brachial arterial pressure(bBP) and central arterial pressure(c BP) is consistent with age,better consistency in DBP,and central arterial pressure is more sensitive than brachial arterial pressure; brachial artery systolic blood pressure(b SBP) is highter than the central artery systolic blood pressure(cSBP),differential value is(15.15 ± 10.20) mm Hg(P〈0.01),brachial artery diastolic pressure(b DBP) is lower than the central artery diastolic pressure(cDBP),differential value is(1.68 + 1.68) mm Hg(P〈0.01),brachial pulse pressure(b PP) is greater than the center pulse pressure(cPP),differential value is(16.94 ± 10.67) mm Hg(P〈0.01).According to CBP multiple stepwise regression analysis,the main factors influencing the b PP are age,heart rate,and height.Conclusion Each index change in brachial arteries and central arterial pressure is consistent with age,generally on the rise,the DBP consistency is good.The central arterial pressure is more sensitive than brachial arterial pressure; The changes in central arterial pressure and brachial arterial pressure have correlation with height,weight,heart rate,and age.
