阿奇霉素与红霉素治疗小儿支原体肺炎的疗效及对免疫球蛋白和T淋巴细胞亚群的影响 被引量:13

摘要 目的:探讨阿奇霉素与红霉素治疗小儿支原体肺炎的临床治疗效果及其对免疫球蛋白和T淋巴细胞亚群的影响。方法:选取2012年11月-2013年11月本院收治的108例小儿支原体肺炎患儿,按随机数字表法将其分为观察组和对照组,每组各54例。两组均给予止咳、退热等常规综合治疗,在此基础上,对照组采用红霉素进行治疗,观察组采用阿奇霉素进行治疗。治疗4周后,比较两组的临床疗效及不良反应的发生情况。结果:观察组总有效率为96.30%,显著高于对照组的79.63%(P<0.05);观察组患儿退热时间、止咳时间、肺部啰音消失时间和平均住院时间均明显少于对照组(P<0.05);观察组患儿Ig A、Ig G、Ig M、CD4+和CD4+/CD8+比值水平显著高于对照组,CD8+水平显著低于对照组,差异均具有统计学意义(P<0.05);观察组不良反应发生率为5.56%,显著低于对照组(P<0.05)。结论:阿奇霉素治疗小儿支原体肺炎疗效显著,能够有效改善患儿的临床症状和体征,且不良反应较小,安全性高,值得临床大力推广和应用。 Objective:To investigate efficacy of azithromycin and erythromycin in treatment of mycoplasma pneumonia and the effects on immunoglobulins and T lymphocyte subsets.Method:108 cases of mycoplasma pneumonia in children were selected from our hospital during the period from November 2012 to November 2013,they were randomly divided into observation group and control group in accordance with the treatment order, 54 cases in each group, two groups of children were given cough, fever and other routine treatment, on this basis, the control group were treated with g erythromycin, the observation group were treated with azithromycin, after 4 weeks of treatment, clinical efficacy and adverse reactions of two groups were compared.Result: Total efficiency of the observation group was 96.30%,significantly higher than the 79.63% in control group(P〈0.05).Cooling time,time cough,pulmonary rales disappeared time and average length of stay of the observation group were significantly less than the control group(P〈0.05).IgA,IgG,IgM, CD4+ and CD4+/CD8+ ratio level of the observation group were significantly higher the control group(P〈0.05).The incidence of adverse reactions of the observation group was 5.56%,significantly lower than the control group(P〈0.05). Conclusion:Azithromycin in treatment of mycoplasma pneumonia have significant effect, can effectively improve the clinical symptoms and signs, and the adverse reaction is small, safe, worth promoting and applications in clinical.
