阿尔茨海默病患者脑电功率值特征分析 被引量:12

The characters of electroencephalogram power in patients with Alzheimer disease

摘要 目的探讨阿尔茨海默病(AD)患者脑电功率值变化特点。方法对30例AD患者和30例健康对照人群进行定量脑电图功率谱分析,记录安静闭目状态下的16导脑电信号,经快速傅里叶变换分别计算δ、θ、α、β频段绝对功率、相对功率以及δ+θ/α+β比值。结果在Fp1、Fp2、F3、F4、C3、C4、P3、P4、O1、O2、F7、F8、T3、T4、T5、T6导联,δ+θ/α+β比值AD组[(1.18±0.27),(1.11±0.30),(1.05±0.36),(1.03±0.31),(1.00±0.33),(1.06±0.36),(0.99±0.38),(1.04±0.37),(0.97±0.37),(1.05±0.38),(0.99±0.33),(1.10±0.23),(0.91±0.28),(0.93±0.32),(1.00±0.40),(0.95±0.34)]高于对照组[(0.65±0.19),(0.63±0.09),(0.59±0.13),(0.55±0.11),(0.54±0.18),(0.49±0.12),(0.50±0.19),(0.47±0.13),(0.40±0.21),(0.40±0.14),(0.61±0.18),(0.59±0.14),(0.59±0.19),(0.54±0.16),(0.45±0.18),(0.50±0.18)],差异有统计学意义(P<0.05)。AD组θ频段相对功率值在Fp1、Fp2、F3、F4、C3、C4、P3、P4、O1、O2、F7、F8、T3、T4、T5、T6导联高于对照组,差异有统计学意义(P<0.05)。AD组δ频带相对功率值在Fp2、F4、C4、P3、P4、O1、O2、F7、F8、T5导联显著高于对照组(P<0.05)。AD组α频带相对功率值在Fp1、Fp2、F3、F4、C3、C4、P4、O1、O2、F7、F8、T3、T5、T6导联低于对照组(P<0.05),AD组θ频带绝对功率值在Fp1、Fp2、F8、T4、P4导联显著高于对照组(P<0.05),AD组α频带绝对功率值在Fp1、F3、O1、O2、F7导联显著低于对照组(P<0.05),AD组β频带绝对功率值在P3、O1、O2、T5导联AD组显著低于对照组(P<0.05)。结论 AD患者定量脑电图指标与正常对照组显著不同,定量脑电图功率谱分析有助于AD诊断。 Objective To study the change of electroencephalogram (EEG)power in patients with Alzheimer's disease(AD) .Methods Thirty patients with AD and thirty healthy people were included.EEG was recorded during eyes-closed resting condition and was transformed to provide estimate for total power, absolute and relative power in delta, theta, alpha and beta activity frequency bands and δ+θ/α+β power radio.Results The power radio of δ+θ/α+β at Fpl,Fp2,F3,F4,C3,C4,P3,P4,O1,O2,FT,F8,T3,T4, TS,T6 in group AD[ (1.18±0.27), ( 1.11±0.30), (1.05±0.36), ( 1.03±0.31), ( 1.00-±0.33), ( 1.06± 0.36), (0.99±0.38) ,( 1.04±0.37), (0.97±0.37), ( 1.05±0.38), (0.99±0.33), ( 1.10±0.23), (0.91± 0.28), (0.93±0.32), ( 1.00±0.40), (0.95±0.34) ] were significantly higher than those in controls [ (0.65± 0.19), (0.63±0.09), (0.59±0.13), ( 0.55±0.11 ), (0.54±0.18), (0.49±0.12), (0.50±0.19), (0.47± 0.13), (0.40±0.21), (0.40-±0.14), (0.61±0.18), (0.59±0.14), (0.59±0.19), (0.54±0.16), (0.45± 0.18) , (0.50±0.18) ] (P 〈0.05).The power of relative band theta at Fpl ,Fp2,F3,F4,C3,C4,P3,P4,O1 O2 ,F7 ,F8 ,T3 ,T4,T5 ,T6 in group AD were significantly higher than those in controls (P 〈0.05).The power of relative band delta at Fp2, F4, C4, P3, P4, O 1,O2, F7, F8, T5 in group AD were significantly higher than those in controls (P 〈0.05, P 〈0.01 ) .The power of relative band alpha at Fpl, Fp2, F3, F4, C3, C4, P4, O1, O2, F7, F8,T3,T5,T6 in group AD were significantly lower than those in controls (P〈 0.05 ). The power of band theta at Fpl, Fp2, F8 ,T4, P4 in group AD were significantly higher than those in controls (P 〈0.05). The power of band alpha at Fpl, F3, O 1,O2, F7 in group AD were significantly lower than those in controls (P 〈0.05).The power of band beta at P3, O1, O2,T5 in group AD were significantly lower than those in healthy people(P 〈0.05).Conclusions The quantiative electroencephalography (QEEG)of patients with Alzheimer disease is significantly different from controls, QEEG is helpful to diagnose AD.
