输尿管软镜下钬激光切开内引流治疗肾盂旁囊肿的疗效分析:附视频

Analysis of efficacy of holmium laser flexible ureteroscopic intrapelvic drainage in the treatment of parapelvic cyst

目的探讨输尿管软镜下钬激光切开内引流治疗肾盂旁囊肿的优势、疗效及安全性。方法分析2010年10月至2014年8月行输尿管软镜下钬激光切开内引流治疗的15例肾盂旁囊肿患者临床资料。患者年龄（39-72）岁,平均53岁,男10例,女5例。均为单侧肾盂旁囊肿。囊肿直径（4-7）cm。囊肿特点：囊肿挤压肾盂、肾盏,囊肿表面正常肾皮质较厚。结果 15例患者手术均成功完成,无中转开放手术。手术时间（25-56）min,平均38 min;术后住院（5-7）d,平均6 d。术后定期随访6个月-2年,12例囊肿消失,3例囊肿直径较术前缩小1/2以上。结论对此种类型的肾盂旁囊肿,输尿管软镜下钬激光切开内引流术操作简单,创伤小、安全、有效,可作为该类肾盂旁囊肿的首选方法。

Objective To explore the superiority, efficacy and safety of holmium laser flexible ureteroscopic intrapelvic drainage in the treatment of parapelvic cyst. Methods The clinical data of 15 patients treated with flexible ureteroscopic incision and drainage using holmium laser between October 2010 and August 2014 were reviewed. This study including 10 male and 5 female patients, ranging from 39 to 72 years old, with an average of 53 years old. All of the parapelvic cysts were unilateral and the diameters were （4-7） cm. The cysts had the characteristics of squeezed renal pelvis and thick cortex of the surface. Results All patients were operated successfully without conversions to open surgery. The average time of operation was 38 min （range： 25-56 min）. The average of postoperative hospital stay was 6 days （range： 5-7 days）. After a follow-up of 6-24 months, the cysts disappeared in 12 patients and reduced by more than one half in 3 patients. Conclusions Holmium laser flexible ureteroscopic intrapelvic drainage is a simple, minimally invasive, safe and effective method for this type of parapelvic cyst, and could be used as the preferred option.
