根管治疗中不同方法测量根管工作长度的效果比较 被引量:4

摘要 目的探讨比较根管治疗中不同方法测量根管工作长度的效果。方法选择2011年6月-2014年7月医院收治中切牙至第二前磨牙的牙髓炎、根尖周炎、残冠、残根、牙折的病例中,从未接受过根管治疗且牙科X线片提示根管通畅的患者70例(98个根管),分别采用根尖X线片法、痛感法、手感法及电测法测量根管工作长度,并与根管插扩大针所测根管工作长度进行比较。结果根尖X线片法准确率15.3%(从X线片直接测量未换算),痛感法准确率25.5%,手感法中15号与20号不同型号扩大针的准确率分别为38.8%和52.0%,电测法准确率92.9%。对于80例根尖有阴影根管根尖孔周围病变X线片上的阴影直径过1.5mm以上时,痛感法和手感法所测和数值的准确率急剧下降,所测数据明显偏大。根尖病变对电测法的准确率影响明显偏小。结论常规根尖X线片和痛感法只能作为测量根管工作长度的辅助方法;手感法准确率强于前2者但单独使用临床效果欠佳;电测法准确性很高,牙髓状况、根尖周病变范围和根管渗出对电测法影响较小,电测法在临床上较可靠,可以推广使用。
