广泛性子宫切除术后下尿路形态的影像学研究

摘要 目的利用超声观察广泛性子宫切除术患者术前及术后下尿路的形态变化,从而评估其解剖结构的改变。方法因子宫病变行广泛性子宫切除术的术后患者45例,并与术前1天及术后3天分别测量静止期及压力期(Valsalva呼吸时)膀胱逼尿肌厚度、膀胱颈移动度(UVJ-M)、膀胱尿道后角(PUVA)及膀胱残余尿量。结果广泛性子宫切除术后患者下尿路解剖结构发生明显改变,UVJ-M与术前比较差异均有统计学意义(P<0.05);膀胱逼尿肌厚度、PUVA及残余尿量与术前比较差异均有统计学意义(P<0.01)。结论超声能够对广泛性子宫全切术后女性下尿路形态学变化进行实时观察、客观评价其解剖结构的改变。 OBJECTIVE By the use of uhrasonography in patients before and after radical hysterectomy surgery and morphological changes in the urinary tract under surgery, in order to assess the changes in their anatomy. METHODS There were 45 cases had rad- ical hysterectomy due to uterine lesion, and the parameters were measured in the preoperative 1 day and 3 days after surgery in qui- escent period and pressure period(when Valsalva breathing), the parameters including: detrusor thickness, bladder neck mobili- ty(UVJ-M), bladder and urethra posterior horn(PUVA) and residual urine volume. RESULTS The urinary tract significantly changed with the patients under extensive resection of uterine anatomy, compared the preoperative parameters, there were statistically signifi- cant differences postoperative with UVJ-M(P〈0.05); there were statistically significant differences postoperative with detrusor thick- ness, bladder neck mobility(UVJ-M), bladder and urethra posterior horn(PUVA) and residual urine volume(P〈0.01). CONCLUSION Ultrasound could be generalized to the urinary tract morphological changes after hysterectomy women under real-time observation, obiective evaluation of chanzes in its anatomy.
