推广品管圈活动 持续改进病案质量

PROMOTION OF QUALITY CONTROL CIRCLE IN THE CONTINUOUS IMPROVEMENT OF MEDICAL RECORDS QUALITY

目的通过尝试在病案质量持续改进中推广品管圈活动,以进一步改进和提高病案质量。方法从2013年1月~2014年12月在各临床科室推广品管圈实践活动,以科室为单位组圈,以品管圈活动的＂十个步骤＂具体要求自行组织品管圈活动。一个季度为一个活动周期,每个周期结束后进行讨论汇总、评估和反馈,总结活动开展情况,探索品管圈活动在改进病案质量中所取得的经验。结果经过两年的实践,病历甲级率由品管圈活动前的94.8%提高到品管圈活动后的99.8%,无丙级病历。结论在病案质量持续改进中开展品管圈活动是可行的,可进一步规范病历书写,提高病案质量,保障医疗质量和医疗安全。

Objective To further improve and enhance the quality of medical records by promoting quality control circle（ QCC）. Methods From January 2013 to December 2014,QCC was promoted in different clinical departments. Every department was served as a circle and the QCC activities were organized based on the specific requirements of ＂ Ten Steps＂ of QCC activities. At the end of the activity cycle,a quarter,the development of the activity was analyzed by discussing,summarizing,evaluating and feeding back to explore the experience of QCC in the improvement of medical records quality. Results After two years of the practice of QCC,the rate of class A record was developed from previous 94. 8% to 99. 8%,without class C record. Conclusion QCC in the continuous quality improvement of medical records is feasible,for it can further standardize the medical record writing,improve the medical record quality,and ensure the medical quality and medical safety.
