布病血清学检测RBPT与SAT一致性研究

Research on the consistency of RBPT and SAT serological test of brucellosis

目的寻求快速、简便、准确的布鲁杆菌病临床诊断方法。方法采用布鲁杆菌病虎红平板凝集试验(RBPT)和试管凝集试验(SAT),对疑似布鲁杆菌病患者血清进行同步检验。结果用RBPT和SAT 2种方法同时检测疑似布病患者血清524份,RBPT的阳性率为21.18%(111/524),SAT的阳性率为22.14%(116/524)。在413份RBPT阴性的血清中,有7份SAT阳性,假阳性率为1.69%。RBPT与SAT总体阳性进行比较,敏感度为93.97%,特异度为99.51%。经一致性检验,K值为0.95,2种试验方法的一致性很高;RBPT与SAT 1∶100++及以上阳性者比较,K值为0.60,2种试验方法的一致性不高。结论 RBPT的假阳性率很高,不能替代SAT用于布病临床诊断。

Objective To explore a fast, simple, and accurate method of the clinical diagnosis of Brucellosis. Methods Sus- pected Brucellosis patients with synchronous were tested by brucellosis rose bengal plate test （RBPT） and serum agglutination test （SAT）. Results Check the 524 serum samples of suspected brucellosis with two methods of RBPT and SAT at the same time, then the positive rate of RBPT was 21.18% （ 111/524）, the positive rate of SAT was 22.14% （116/524）. In 413 nega- tive serum samples of RBPT, there were 7 positive, the false positive rate was 1.69%. The positive comparison between RBPT and SAT, the sensitivity was 93.97% , the specificity was 99.51% , the value of K was 0.95 with conformance test, the two kinds of consistency test had high rate ; compared with RBPT. SAT 1 ： 100 ＋ ＋ and the above of positives, the value of K was 0.60, the rate of the two consistency kinds of tests was not high. Conclusion The false rate of RBPT has high rate, it can＇ t replace SAT to clinically diagnosis of Brucellosis.
