心电图与冠脉造影对冠心病不稳定性心绞痛诊断结果的比较研究

A comparative study of ECG and coronary angiography in diagnosis of Unstable angina results

目的研究比较心电图与冠脉造影对冠心病不稳定性心绞痛的诊断结果。方法选取我院心内科收治的冠心病不稳定性心绞痛患者186例,所有患者给予常规治疗,于患者心绞痛发作时利用12导联心电图机进行检查,记录分析心电图ST-T改变的情况;入院一周内行冠状动脉造影,观察动脉血管病变;比较心电图与冠脉造影的诊断结果。结果 1心电图检查示前壁缺血患者90例,血管病变率LAD(86.6%)>LCX(13.4%)>LMCA(0%)、RCA(0%),差异有统计学意义(P<0.05);2心电图检查示下壁缺血患者24例,血管病变率RCA(62.5%)>LAD(25.0%)>LCX(12.5%)>LMCA(0%),差异有统计学意义(P<0.05);3心电图检查示侧壁缺血患者33例,血管病变率LCX(54.5%)>LAD(27.3%)>RCA(18.2%)>LMCA(0%),差异有统计学意义(P<0.05)4心电图检查示心肌广泛缺血患者39例,血管病变率LMCA(61.5%)>LAD(30.8%)>LCX(7.7%)>RCA(0%),差异有统计学意义(P<0.05)。结论心电图对冠心病不稳定性心绞痛血管病变的定位诊断,与冠脉造影结果相符,对临床具有指导意义,值得临床推广。

Objective To compare the results of ECG and coronary angiography in diagnosis of coronary heart disease unstable angina pectoris.Methods Coronary heart disease in our hospital were selected in Department of Cardiology,186 cases with unstable angina patients,all patients were given conventional treatment,in patients with angina pectoris attacks by 12 lead electrocardiogram machine inspection,recording and analysis of electrocardiogram ST-T change sitUAPtion;admission within one week coronary arteriography,observe the arterial lesion;diagnosis results compared with ECG and coronary angiography.Results ① The electrocardiogram in 90 cases of anterior wall ischemia in patients,the vascular disease rate is LAD（86.6%）〉LCX（13.4%）〉LMCA（0%）,RCA（0%）,the difference was statistically significant （P〈 0.05）;②The ECG examination showed 24 cases of patients with inferior wall ischemia,vascular disease rate of RCA RCA（62.5%）〉LAD（25.0%）〉LCX（12.5%）〉LMCA（0%）,the difference was statistically significant （P〈0.05）;③The ECG examination showed 33 patients with side wall ischemia,vascular disease rate of LCX（54.5%）〉LAD（27.3%）〉RCA（18.2%）〉LMCA（0%）,the difference was statistically significant （P〈0.05）;④The ECG examination showed 39 patients with extensive myocardial ischemia,vascular lesions rate of LMCA （61.5%）〉LAD（30.8%）〉LCX（7.7%）〉RCA（0%）,the difference was statistically significant（P〈0.05）.Conclusion The electrocardiogram in diagnosis of coronary heart disease unstable angina pectoris of vascular disease and coronary angiography results of important significance in guiding clinical,is worth the clinical promotion.
