腹腔镜手术与传统开腹手术在结直肠癌根治术中的临床疗效比较

Comparison of the clinical efficacy of laparoscopic colorectal cancer radical operation and traditional open operation

目的 对比腹腔镜下结直肠癌根治术与传统开腹手术的临床疗效.方法 选取2009年2月～2013年12月我院普外科收入院的155例结直肠癌患者分为对照组77例和观察组78例,对照组采用传统开腹手术,观察组采用腹腔镜下结直肠癌根治术,对比观察两组患者的手术时间、术中出血量、住院时间、淋巴结清除数、术后并发症和满意度情况,分析两组患者的临床疗效.结果 观察组78例患者的术中出血量为（102＆#177;24.4）ml,住院时间为（7＆#177;2.8）d,对照组患者的术中出血量为（188＆#177;57.9）ml,住院时间为（13＆#177;6.2）d,观察组明显优于对照组,差异有统计学意义（P＜0.05）;两组患者的手术时间、淋巴结清除数和并发症发生率比较,无统计学差异（均P＞0.05）;两组患者治疗满意度分别为91.03％和77.92％,观察组明显优于对照组,差异有统计学意义（P＜0.05）.结论 腹腔镜下结直肠癌根治术与传统开腹手术相比,是一种创伤小、出血少、恢复快的有效方法,值得临床推广应用.

Objective To comparatively investigate the clinical efficacy of the laparoscopic colorectal cancer radical operation and the traditional open operation. Methods 155 colorectal cancer patients were divided into control group （77 cases） and the observation group （78 cases）. The control group was treated by traditional open operation, and the obser- vation group was treated by laparoscopic colorectal cancer radical operation. The operation time, intraoperative blood loss, hospital length of stay, the number of cleared lymph node, postoperative complications, satisfaction, and clinical ef- ficacy were observed. Results Of observation group, the intraoperative blood loss was （102±24.4）ml and the hospital length of stay was （7±2.8）days. Of control group, the intraoperative blood loss was （188± 57.9） ml and the hospital length of stay was （13±6.2） days. The observation group was better than the control group（P〈0.05）, the difference has statistics significance. The operation time, the number of cleared lymph node and the postoperative complications of two groups（P〈0.05） had no statistical difference. The satisfaction of the two groups were 91.03% and 77.92%, the observation group was better than the control group（P〈0.05）, the difference has statistics significance. Conclusion Compared with the traditional open operation, laparoscopic colorectal cancer radical operation is a kind of effective opera- tion methods for the treatment of colorectal cancer with small trauma, less bleeding, faster recovery, worthy to be widely promoted and applied clinically.
