尿激酶预防PICC堵管的效果观察

Effect of Urokinase on Prevention of PICC Obstruction

目的探讨尿激酶预防PICC堵管的应用效果。方法 2013年1月—2014年12月选取我院置入PICC的肿瘤患者160例,按随机数字表法分为观察组(n=80)与对照组(n=80)。对照组给予PICC封管常规护理,观察组在对照组基础上给予尿激酶稀释液冲管,每周1次。观察比较两组患者PICC堵管发生率、非计划性拔管发生率及留置时间。结果观察组PICC堵管发生率、非计划拔管发生率低于对照组,PICC留置时间长于对照组,差异均有统计学意义(P<0.01)。结论使用尿激酶稀释液冲管每周1次,能有效预防肿瘤患者PICC堵管发生,延长留置时间,为临床治疗提供了方便,值得推广应用。

Objective To investigate the effect of urokinase on the prevention of PICC obstruction. Methods A total of 160 cases of tumor patients with PICC from January, 2013 to December, 2014 were randomly divided into observation group （n=80） and control group （n=80）. Routine care for PICC sealed tube was performed in control group while in observation group, urokinase dilution was used to wash the tube every week in addition to the routine care. The incidence of PICC obstruction, the unscheduled decannulation rate and the retention time of PICC in two groups were compared. Results The ineidence of PICC obstruction and the unscheduled decannulation rate were lower in observation group than those in control group but the retention time of PICC was longer than that of control group. All differences were statistically significant （P〈0.01）. Conclusion Washing the tube of PICC by urokinase dilution once a week can prevent PICC obstruction effectively for tumor patients and extend the catheter＇s retention time. It provides a convenient method for clinical treatment and reduces the pain as well as economic burden for the patients.
