儿童肱骨髁上骨折标准化复位手法临床研究

Clinical Study on the Application of Normalized Reduction Manipulation in Children’s Supracondylar Fracture of Humerus

目的：评价标准化手法整复治疗儿童肱骨髁上骨折的近中期疗效及安全性。方法：将80例肱骨髁上骨折患儿随机分为试验组（40例）和对照组（40例），试验组采用标准化复位手法配合经皮克氏针固定手术治疗，对照组采用常规手法整复治疗，比较两组整复时间，术后24h、48h、72h肘关节VAS评分，采用Broberg-Morrey功能评分评估术后1个月肘关节形态、活动度情况。结果：试验组整复时间短于对照组（P〈0.05），试验组术后24h、48h、72h肘关节VAS评分均低于对照组（P〈0.05）；试验组关节功能优良率高于对照组（P〈0.05）。结论：标准化复位手法配合经皮克氏针固定手术具有整复时间短、患儿疼痛缓解明显、肘关节形态及活动度恢复佳等特点，值得临床推广应用。

Objective：To evaluate the short-term and mid-term efficacy and safety of the normalized reduction manipulation for children＇s supracon-dylar fracture of humerus. Methods：80 cases of children with supracondylar fracture of humerus were randomly divided into experimental group （40 cases） and control group （40 cases）, the experimental group treated by normalized reduction manipulation combined with Percutaneous Kirschner wire fixation operation, while control group by conventional twisted taxis, compared two groups＇ taxis time, VAS score of elbow at 24h, 48h, 72h af-ter surgery, and used Broberg-Morrey functional scores to evaluate the shape and range of elbow at 1 mouth after surgery. Results：The taxis time of experimental group was shorter than that of control group （P〈0.05）, the VAS score of elbow at 24h, 48h,72h after surgery were all lower than those of control group （P〈0.05）, the excellent rate of joint function was higher than that of control group （P〈0.05）. Conclusion：The treatment of normal-ized reduction manipulation combined with Percutaneous Kirschner wire fixation operation has some advantages such as short taxis time, significant relief of children＇s pain, good recovery of elbow＇s shape and range, deserves clinical promotion and application.
