低浓度对比剂低辐射剂量及ASIR行冠状动脉CT血管造影最适迭代权重的临床研究 被引量:2

Clinical Study on the Best Image Quality of Different Percentage ASIR Using 100kVp and 270 mgI/ml Iodixanol in Coronary Artery CTA

摘要 目的比较不同权重自适应统计迭代重建技术(ASIR)在低浓度对比剂及低辐射剂量条件下行冠状动脉CT血管造影(CTA)的图像质量。方法收集2014年3月至2014年8月间疑诊为冠心病并行冠状动脉CTA检查患者90例,体质指数(BMI)在20~25 kg/m2之间。行双低条件下(100 k Vp及270 mg I/ml碘克沙醇对比剂)及前瞻性心电门控技术行冠状动脉CTA扫描。扫描完成后对图像进行不同权重ASIR(0、20%、40%、60%、80%、100%),并对图像质量进行主观及客观分析。结果相对于滤波反投影法(FBP),20%、40%、60%、80%、100%ASIR图像噪声分别降低12%、24%、33%、46%、51%,差异有统计学意义(P〈0.01);信噪比、对比噪声比分别提高约7%、22%、50%、81%、103%及8%、23%、50%、81%、101%。不同权重ASIR重建图像CT值差异无统计学意义。0、20%、40%、60%、80%、100%ASIR的主观评分分别为1.5、1.9、3.9、4.0、2.0和1.2分,差异有统计学意义(P〈0.05)。权重设置在40%、60%ASIR图像质量具有最高主观评分,冠状动脉图像质量最好。结论在双低条件下及ASIR重建技术行冠状动脉CTA检查,相对于FBP重建算法,不同权重ASIR图像在40%、60%图像质量具有最高主观评分,图像质量最好。 Objective To compare the image quality of different percentage ASIR using 100 k Vp and 270 mg I / ml iodixanol in coronary artery CT angiography( CTA) with 64 slice spiral CT scanner. Methods Totally 90 patients whose BMI were20 to 25 kg / m2 with suspected coronary disease underwent coronary artery CTA from March 2013 to August 2013. Coronary artery CTA scanning parameters and group information were as follows: 100 k Vp,600 m A; contrast media: Visopaque270 mg I / ml. Raw data was reconstructed with different percentages of ASIR( 0,20%,40%,60%,80%,100%). Results In comparison with FBP( 0 ASIR),the use of 20%,40%,60%,80% and 100% ASIR resulted in reduced image noise between groups(- 12%,- 24%,- 33%,- 46% and- 51% respectively; P〈0. 01) without difference in the mean CT attenuation of the measured vessels. ASIR also increased SNR and CNR between groups( 7%,22%,50%,81%,103% and8%,23%,50%,81%,101%,respectively; P〈0. 01). There were significant differences between groups( 0,20%,40%,60%,80% and 100% ASIR) in the subjective scores( 1. 5,1. 9,3. 9,4. 0,2. 0 and 1. 2 respectively; P〈0. 01). Reconstruction using 40% and 60% ASIR had the highest subjective scores and the best imaging quality for coronary artery. Conclusion In comparison with FBP,reconstruction using 40% and 60% ASIR had the highest subjective scores of imaging quality in coronary artery CTA.
