Bakri球囊联合Zheng子宫压迫缝合术治疗产后出血的临床效果观察

Observation on the clinical effect of Bakri balloon tamponade combined with Zheng uterine compression suture in treatment of postpartum hemorrhage

目的：探讨Bakri球囊联合Zheng子宫压迫缝合术治疗产后出血的临床疗效。方法：回顾性分析2006年1月～2014年12月解放军第174医院产科剖宫产产后出血的300例患者资料,其中Bakri球囊组100例、Zheng子宫压迫缝合术组95例、Bakri球囊联合Zheng子宫压迫缝合术组105例。比较3组治疗的有效性及术中和术后出血量。结果：Bakri球囊联合Zheng子宫压迫缝合术组的止血效果优于Zheng子宫压迫缝合术组及Bakri球囊组（P〈0.05）,Zheng子宫压迫缝合术组与Bakri球囊组相比,差异无统计学意义（P〉0.05）。Bakri球囊联合Zheng子宫压迫缝合术组的术中及术后24 h出血量少于其他两组（P〈0.05）,Zheng子宫压迫缝合术组与Bakri球囊组的术中出血量相比,差异无统计学意义（P〉0.05）。Zheng子宫压迫缝合术组术后24 h出血量少于Bakri球囊组（P〈0.05）。结论：Bakri球囊联合Zheng子宫压迫缝合术治疗产后出血的疗效优于二者单独使用的疗效。

Objective： To explore the clinical effect of Bakri balloon tamponade combined with Zheng uterine compression suture in treatment of postpartum hemorrhage. Methods： The data of 300 cases with postpartum hemorrhage after cesarean section in Department ofObstetrics in the hospital from January 2006 to December 2014 were analyzed retrospectively, then they were divided into Bakri balloon tamponade group （ 100 cases）, Zheng uterine compression suture group （95 cases）, and combination therapy group （ 105 cases, Bakri balloon tamponade combined with Zheng uterine compression suture） . The clinical effects, introperative and postoperative amounts of blood loss in the three groups were compared. Results： The hemostatic effect in combination therapy group was statistically significantly better than those in Bakri balloon tamponade group and Zheng uterine compression suture group （ P〈0. 05 ）, but there was no statistically significant difference between Bakri balloon tamponade group and Zheng uterine compression suture group （P〉0. 05 ） . The amounts of blood loss during operation and at 24 hours after operation in combination therapy group were statistically significantly less than those in the other two groups （P〈0. 05 ）, there was no statistically significant difference in the amount of blood loss during operation between Bakri balloon tampenade group and Zheng uterine compression suture group （ P〉0.05 ） . The amount of blood loss at 24 hours after operation in Zheng uterine compression suture group was statistically significantly less than that in Bakri balloon tamponade group （P〈0. 05 ） . Conclusion： The clinical effect of Bakri balloon tamponade combined with Zheng uterine compression suture in treatment of postpartum hemorrhage is better than separate application of the two methods.
