四川北部地区肠杆菌科细菌产ESBLs CTX-M亚型研究 被引量:2

摘要 目的了解川北地区临床分离的大肠埃希菌和肺炎克雷伯菌中产超广谱β-内酰胺酶(ESBLs)CTX-M型亚型的分布情况。方法选取CTX-M型的42株大肠埃希菌和14株肺炎克雷伯菌作为研究对象,对PCR扩增CTX-M耐药基因阳性者行DNA测序,测序结果在Genebank中进行比对。结果 42株大肠埃希菌中有30株出现CTX-M基因阳性条带,6株弱阳性;14株肺炎克雷伯菌,5株出现CTX-M阳性条带,2株弱阳性。ESBLs分子表型流行以CTX-M-14型为主,其次为CTX-M-3型,且发现1株OXY-2型;VITEK AES耐药表型判断的准确性达76.8%。结论川北地区ESBLs分子表型中CTX-M-14型流行情况严重,应进一步加强消毒隔离措施和流行菌株的控制。 Objective To investigate north area the clinical isolates of escherichia coli and klebsiella pneumoniae producing extended spectrum B- lactamases (ESBLs) distribution of CTX-M-type subtype. Methods Selected 42 CTX-M-type escherichia coli and 14 klebsiella pneumoniae as the research object, CTX-M-positive resistance gene was ampliifed by PCR for DNA sequencing, sequencing results were compared in the Genebank. Results 30 CTX-M gene positive bands appeared in 42 E.coli, 6 had weakly positive, 5 appear CTX-M positive bands in 14 klebsiella pneumoniae, 2 had weakly positive. ESBLs molecular phenotype popular with CTX-M-14 type, followed by the CTX-M-3 type, and found an OXY-2 type, VITEK AES resistant phenotype to determine the accuracy of up to 76.8%. Conclusion The North Region ESBLs molecular phenotype in CTX-M-14-type epidemic situation is serious, it should further strengthen disinfection and isolation measures and control of the epidemic strain.
