不同联合手术方式治疗青光眼合并白内障效果研究

Research on the efficacy of glaucoma with cataract surgery combined with different ways of treatment

目的探讨人工晶体植入术、青光眼小梁切除术联合白内障超声乳化摘除术治疗青光眼合并白内障的临床效果。方法选取2009年12月至2014年4月本院收治的青光眼合并白内障患者82例,按照患者就诊时间分为观察组与对照组,每组各41例。观察组患者行人工晶体植入术、青光眼小梁切除术及白内障超声乳化摘除术治疗,对照组患者行人工晶体植入术、青光眼小梁切除术及白内障囊外摘除术治疗。观察并比较两组患者术后1、2、4周视力、眼压、散光度。结果观察组患者手术减压有效率为95.1%(39/41),对照组为92.7%(38/41),两组比较差异无显著性(P>0.05)。观察组患者术后1、2、4周视力均显著优于对照组,散光度明显小于对照组,差异均具有显著性(P<0.05,P<0.01)。结论人工晶体植入术、青光眼小梁切除术联合白内障超声乳化摘除术治疗青光眼合并白内障疗效确切,能够显著提高患者视力,降低散光度,值得临床推广应用。

Objective Study the clinical effect of artificial lens implantation, trabeculectomy and phacoemulsiifcation in the treatment of glaucoma complicated with cataract. Method Chose 82 patients in our hospital from December 2009 to April 2014 for treatment of glaucoma complicated with cataract, according to the time patients were divided into control group and observation group, 41 cases in each group. The observation group underwent artificial lens implantation, trabeculectomy and phacoemulsification treatment, the control group underwent artiifcial lens implantation, trabeculectomy and extracapsular cataract extraction. Observed and compared vision, astigmatism, intraocular pressure of the two groups after operation 1, 2, 4 weeks. Result Patients in observation group operation of decompression efifciency was 95.1%（39/41）, control group was 92.7%（38/41）, the difference between the two groups was not signiifcant （P 〉 0.05）. Visual acuity of observation group after 1, 2, 4 weeks was signiifcantly better than control group, the astigmatism was signiifcantly less than control group, the difference was signiifcant （P 〈 0.05, P 〈 0.01）. Conclusion Artiifcial lens implantation, trabeculectomy and phacoemulsification treatment for glaucoma complicated with cataract compared with conventional incision method, can signiifcantly enhance the visual acuity, lower astigmatism, worthy of clinical application.
