老年高血压患者血压变异性及血压晨峰的相关因素分析

Analysis of Correlated Factors of Elderly Hypertensive Patients Blood Pressure Variability and Morning Blood Pressure Peak

目的探讨老年高血压患者的血压变异性（BPV）和血压晨峰之间的关系。方法选取我院2012年12月-2014年12月间收治的施行动态血压监测的288例老年高血压患者作为临床研究对象,根据患者的年龄分别将其分为A组（65-69岁）108例、B组（70-74岁）81例、C组（75-79岁）58例、D组（≥80岁）41例。另选我院同期收治的100例〈65岁的高血压患者作为对照组。观察患者的BPV和血压晨峰值,分析各个因素对血压晨峰的影响。结果与A组、B组、C组和对照组相比,D组患者的24 h SBP、24 h DBP、d SBP、n SBP及n DBP的标准差和变异系数均有明显上升,（P〈0.05）差异均有统计学意义。患者的年龄、BMI及血压昼夜模式等因素是血压晨峰的影响因素（P〈0.05）。结论老年高血压患者的BPV随着年龄的增长而逐渐上升,而患者的年龄、BMI及血压昼夜模式是血压晨峰的主要影响因素。

Objective To investigate the variability of blood pressure in elderly patients with essential hypertension （ BPV ） and the relationship between mormng blood pressure peak. Methods 288 cases of elderly patients with hypertension ambulatory blood pressure monitoring performed in our hospital in 2012 December - 2014 years were selected from the clinic in December as research object, according to the age of the patients were divided into A group （ 65 - 69 years old ） in 1（18 cases, B group （ 70 - 74 years old ） in 81 cases, C group （ 75 - 79 years old ）, 5g cases olD group （ over N0 years ）in 4I cases. Choose our hospital admitted 100 cases 〈65 year old hypertensive patients as control group. Observation of patients with BPV and moroing blood pressure peak, analyzed effects of various factors on the morning blood pressure peak. Results With the A g-coup, B group, compared to the C group and the control group, patients in the D group 24 hSBP, 24 hDBP, dSBP, nSBP and uDBP of standard deviation and coefq%ient of variation were significantly increased （P〈0.05）, the difference had statistical significance. Age, BMI and c-rcadian patterns and other factors were influencing factors in patients with morning blood pressure surge （ P〈0.05 ） . Conclusion In elderly patients with hypertension BPV gradually increased with the growth of the age, and the age of patients, BMI and blood pressure circadian pattern is the main influence factors of morning blood pressure surge.
