摘要
目的探讨心肌肌钙蛋白Ⅰ、肌酸激酶同工酶MB及高敏C反应蛋白对病毒性心肌炎的诊断意义。方法收集我院拟诊"病毒性心肌炎"的患者作为研究对象,按患者入院后是否诊断为病毒性心肌炎,分为病毒性心肌炎组和对照组。对比(1)病毒性心肌炎组和对照组心肌肌钙蛋白Ⅰ、肌酸激酶同工酶MB及高敏C反应蛋白水平。(2)计算心肌肌钙蛋白Ⅰ、肌酸激酶同工酶MB及高敏C反应蛋白及三种联合对诊断病毒性心肌炎的灵敏度、特异性。结果 (1)病毒性心肌炎组和对照组心肌肌钙蛋白Ⅰ、肌酸激酶同工酶MB及高敏C反应蛋白水平差异有统计学意义(P<0.05)。(2)心肌肌钙蛋白Ⅰ、肌酸激酶同工酶MB、高敏C反应蛋白及三种联合对诊断胰腺炎的灵敏度、特异性差异有统计学意义(P<0.05),以三种联合诊断灵敏度及特异性最高。结论诊断病毒性心肌炎时,联合心肌肌钙蛋白Ⅰ、肌酸激酶同工酶MB及高敏C反应蛋白检测能更准确的诊断病毒性心肌炎,便于早期诊断、早期治疗。
Objective To investigate the diagnostic significance of cardiac troponin I, creatine kinase isoenzyme MB and high sensitive C reactive protein in viral myocarditis. Methods Collected in our hospital diagnosed as "viral" patients as the research object, according to whether the diagnosis of viral myocarditis patients after admission, divided into viral myocarditis g'roup and control gronp. Contrast ( 1 ) viral myocarditis group and the control group, the levels of cardiac troponin creatine kinase isoenzyme MB and high sensitive C reactive protein. Calculation of cardiac troponin I ( 2 ), creatine kinase isoenzyme MB and high sensitive C reactive protein and three kinds of joint sensitivity in the diagnosis of viral myocarditis, specificity. Results ( 1 ) there was statistical significance in viral myocarditis group and the control group, the levels of cardiac troponin creatine kinase isoenzyme MI] and high sensitive C reactive protein ( P〈O.05 ) difference. ( 2 ) had statistical significance of cardiac troponin 1, creatine kinase isoenzyme MB, high sensitive C reactive protein and three kinds of combined with sensitivity, specificity for the diagnosis of acute pancreatitis (P〈0.05), the differences in three kinds of combined diagnostic sensitivity and specificity of the highest. Conclusion The diagnosis of viral myocarditis, cardiac troponin 1, combined detection of creatine kinase isoenzyme MB and high sensitive C reactive protein in diagnosis of viral myocarditis can be more accttrate, convenient for early diagnosis, treatment.
